Person-Centred Care Planning Using Orem's Model: A Case Study of David

Aim of the Assessment
The aim of this assessment is to enable you to demonstrate your ability to critically appraise the application of a systematic nursing assessment by producing a person-centred care plan focusing on the Goals, outcomes, and interventions of three health needs for a person, using evidence-based practice to evaluate your rationale for your intervention.
The assessment task is to submit a 1500 words report supporting your goals, outcomes and interventions using evidence from literature and one of the three nursing models, provided in this module. A care-plan must be attached as an appendix.
Aim of the Assessment

Additional Information
During the course you have engaged with information and activities concerning David, Malik's partner. For the purposes of this assessment please use your knowledge of David so far and the information provided below to complete your nursing assessment.
Please Use one of the identified nursing models discussed in the course content to help you to formulate a care plan focusing on three David’s Health needs. The care plan should be presented in the appendix using three columns headed 'Health need ', 'Goal and outcome’ and 'Intervention'.
The health problem/Need is the issue or problem identified by the patient for example, Pain in the chest (or other areas).
The goal and outcome are the aim for your intervention and the specific measurable aspect of the aim. For example, For a person with respiratory issues your goal and outcome could be “to maintain oxygen saturation above 88%”
The interventions are the steps agreed between the patient and the health care professional that need to be taken to reach the goal. For example, “I would like the nurse to take a full set of observations at each visit”
David's current issues. Please chose 3 of these issues to focus your assessment on and assume that for the areas not identified below David does not have any current problems.
Increased breathlessness due to Chronic Obstructive Pulmonary Disease (COPD)
David has increased his consumption to more than 20 units of alcohol per week **
David is smoking in excess of thirty cigarettes a day. **
Increased stress and anxiety due to caring role for his partner, Malik who has early stages of dementia
Not sleeping well
** One of the three problems should appraise a related health promotion aspect of care to meet learning outcome 2.
Submission Instructions:
Presentation
The assignment should be presented as a 1500-word report and an attached care plan as an appendix, using the following structure:
Introduction (150 words approx)
Introduction to David and the report and identification of the nursing model used with reference to your completed care plan in your appendix.
Choose one of the following; ‘Roper-logan –Tierney’, ‘Neuman’s’ or ‘Orem’s’ model of Nursing.
Background (200 words approx)
Discussion and critical appraisal of the nursing model used and how this model helped to plan a person-centred care plan
Discussion (1000 words approx)
For each of the three problems identified in your care plan critically discuss the rationale for the interventions provided in the care plan, using an evidence-based approach.
You can present your discussion section as below
Problem 1 (then state the problem from your care plan and discuss as above)
Problem 2 (then state the problem from you care plan and discuss as above)
Problem 3 (then state the problem from you care plan and discuss as above)
Or
Choose a format that is appropriate for your care plan (appendix), chosen Nursing model and interventions.
Conclusion (150 words approx.)
Summarise key points from the entire assignment explaining how these meet learning outcomes 1-3
Care plan -Attached as an appendix.
3 health needs/problems from the list above that you addressed in the report. You must include one of the health promotion aspects highlighted in red.
Goals and outcomes -One goal and outcome for each health need/problem.
Interventions - At least three interventions for each health need/problem.
