The article under review is titled Perioperative risk of redo aortic valve replacement done by Christiansen, Schmid, and Autschbach. The study involves 63 participants comprised of 53 males and ten females. The study took place between 2001 and 2005 where the 63 participants underwent isolated redo aortic valve replacement (AVR). The study involved two groups of patients (Group 1 and 2). Group 1 included only the patients with an isolated primary AVR from where an isolated redo AVR was performed forming group 2. Data collection involved viewing patients’ charts.
The study carried out by Christiansen, Schmid, and Autschbach (2009) was a retrospective study. The study combined both qualitative and quantitative data in their analysis. A t-test statistical analysis was performed for the dependent variables for the quantitative data whereas the McNemar test was performed for the qualitative data. The SAS software conducted the statistical calculation for the research. For statistical significance, a p-value of less than 0.05 (p< 0.05) was considered to be statistically significant.
The article provides a summary of critical preoperative data. On the result tabulations, one table indicates different reasons for AVR. These results suggest that 22 percent of the patients suffered from a bicuspid aortic valve. 11 group 2 patients suffered from prosthetic endocarditis without any signs of stenosis or insufficiency. Paravalvular leakages caused the insufficiency among the 11 of the 27 patients of group 2 who underwent redo AVR for prosthetic incompetence. The article also provides other critical operative parameters in the results. The parameters include the mean duration of 10 ± 6.5 years from the primary to the redo procedure for all the patients; 23 ±11.3 years for the patients after primary aortic valve reconstruction for four patients; 6.2 ± 6.1 years for 18 patients after mechanical AVR and lastly 10.4 ± 4.0 years for 41 patients after biological AVR.
Fifty-two implantations of the mechanical prosthesis, eight implantations of biological prostheses and three treatments with a reification of the primary implanted prosthesis were conducted during the redo procedures. The article notes that there were no injuries of the heart or the great vessels were inflicted during the redo procedures. The procedure involved instituting a cardiopulmonary bypass via cannulation of the right atrium and the ascending aorta. The article also notes that a left ventricular vent as regularly inserted through the right upper pulmonary vein during the procedures.
