Functional Behavior Assessment Report

Research Paper Instructions:

Functional Behavior Assessment Report ( FBA Report) for ----------------
Using one of the following referral case studies, develop and outline a comprehensive functional behavioral assessment report. The specific objective of your functional behavioral assessment report is to demonstrate synthesis of the course material related to FBA’s as they have been presented in the course. The FBA Report should be specifically related to only one of the following case scenarios.
Choose one of the following case scenarios for your assignment:
Scenario 1
A third-grade student in a public elementary school has been referred for behavior problems continuously for the past three months. This student is demonstrating consistently disruptive behaviors in this classroom and in other settings within the school (lunch, recess, specials, etc.). Disruptive behaviors have been termed “aggressive” and “non-compliant” by school staff.
Scenario 2
Johnny is a six-year-old child with autism. His communication skills are significantly deficient, and he demonstrates a wide variety of problem behaviors. Most concerning is the tendency for him to demonstrate self-injurious behavior that has recently increased in frequency and intensity. An emergency room visit was necessitated by a recent incident of this behavior. Some of Johnny's more intense characteristics related to autism spectrum disorders include repetitive and ritualistic behavior patterns and an apparent need to collect and acquire a variety of specific tangible items, such as Hot Wheels cars and Legos. Johnny is generally only “well behaved” when access to these items is free and frequent.
Assignment Directions
Use Unit 9 Assignment Template as a model for your report and the Unit 9 Assignment Tips, please complete each section of the FBA, You must include specific details as they relate to the referral scenario you are addressing. Details should follow the descriptions under each section heading within the template.
• Identifying Information
o Complete the client information.
• Reasons for Referral
o Discuss why a referral for services is being made.
o Discuss presenting problems/behavioral concerns.
• Relevant Background Information
o Include hypothetical discussion of client’s history including family history/background, prior evaluations conducted, history of problem behavior, and current concerns.
 Creativity should be used here to add to the current scenario.
• Description of Evaluation Procedures
o Indirect Assessment
 List the indirect FBA methods that were used and provide a discussion of each method, including the purpose of each and the type of information each was designed to gather in relation to the case scenario.
 Discuss how each of the indirect methods was carried out during the assessment process.
o Direct Descriptive Assessment
 List the direct FBA methods that were used and provide a discussion of each method, including the purpose of each and the type of information each was designed to gather in relation to the case scenario.
 Discuss how each of the direct methods was carried out during the assessment process (observational data can be included here).
• Behavior(s) Requiring Intervention
o Discuss the target behavior and give an operational definition for each one.
o Current Level of Occurrence
 Discuss the baseline data of the target behavior (how frequently the behavior is occurring) and include other dimensions of behavior as needed.
o Hypothesized Function
 Discuss the results of each of the indirect and direct assessment procedures, including antecedent (SD, MO) and consequence variables that impact the behavior in the scenario.
 Indicate the hypothesis of the function (positive, negative, or automatic reinforcement) of each problem behavior.
• Intervention Recommendations
o Antecedent Recommendations
 Discuss recommendations for interventions that can be utilized to modify the environment to reduce or eliminate the variables that serve to trigger the problem behaviors identified in the scenario.
o Replacement Behavior Recommendations
 Discuss strategies to be utilized to teach and increase behaviors that will serve to replace the problem behaviors identified in the scenario.
o Reaction Procedure Recommendations
 Discuss procedures to be implemented when the problem behaviors occur.
o Progress Monitoring and Behavioral Consultation
 Discuss how data collection tools will be used to continue to monitor response to intervention strategies and overall progress.

Functional Behavior Assessment Report for Johnny
Identifying Information
Name of Client: Johnny Smith
Age/Date of Birth: 4th April, 2017
Parent’s Names: Donald Smith and Mary Stone
School: Oakwood Elementary School Hickory
Date of Report: 5th August, 2023
Reasons for Referral
The client is being referred for demonstrating a broad range of problem behaviors including a penchant for self-injurious behavior, which have recently increased in frequency and intensity. He was recently rushed to the emergency room after harming himself. Johnny’s communication skills are significantly deficient and some of his most intense characteristics are connected to autism spectrum disorders such as repetitive and ritualistic behavior patterns. For instance, the client has a strong urge to acquire and collect different tangible items like Legos and Hot Wheels cars. Johnny is more cooperative when he is allowed free and frequent access to these tangible items.
Relevant Background Information
Johnny’s mother reports that her son began experiencing speech impediments shortly after his fourth birthday. He started avoiding mingling with other children of his age group after his speech regressed. The speech problems were accompanied by several other symptoms including physical restlessness, mood swings, repetitive action, and aggressive behaviors like tantrum-throwing, self-harming, and head-banging. Both parents were perplexed by these symptoms since Johnny’s only sibling (Martin, 10 years) has no history of behavior problems. The parents have never sought any professional help for their son. Johnny is indulged and all his demands have to be met, or else he becomes very angry. Unfortunately, his behavior problems are becoming a challenge at school, especially for his teachers.
Description of Evaluation Procedures
The indirect FBA methods that were used include rating scales and interviews. A behavior rating scale is an evaluation tool completed by a third party or clinician that includes several items that evaluate one or several targeted client behaviors (Saini & Cox, 2020). The client’s parents completed rating scales that were used to screen for behavioral problems and assess the client’s performance as well as social skills (Steege et al., 2019). An interview with the client’s parents was also conducted to gather specific details about his behavioral issues. The interview was useful in understanding the severity and extent of the problem behaviors.
The direct FBA methods that were used were ABC narrative and scatter plot. ABC narrative is a form of direct observation in which antecedent events and consequences are recorded in an open-ended manner. This method was used to gather information on target behaviors. The scatter plot is a direct observation instrument used to identify the occurrence of target behaviors. This method was used to track the client’s problem behaviors, particularly when and how they occur.
