The case is a patient who has difficulty accessing diabetic care. The patient became paralyzed after being involved in a car accident. Before the paralyzed, the patient was diabetic and so the car accident contributed to his problems in accessing care. Given that he has to use a wheelchair to move around, the patient has had difficulty coming in for regular checkups. The patient has difficulties accessing transportation Additionally, he has difficulties perform self-management routines for his diabetes because he is paralyzed from the waist down. He often misses appointments or comes in late for appointments.
One potential adverse outcome that could happen to the patient is developing diabetes complications. Adu et al. (2019) argue getting regular check-ups and proper self-management techniques are vital in preventing diabetes complications. Many patients do get complications from diabetes when it is not managed appropriately (Adu et al., 2019). In the case of the patient, his situation is worsened by the difficulty accessing appropriate care and support. As a patient with paralysis, he needs extra care to manage his diabetes and overall well-being. Complications often arise when regular checkups cannot be conducted at the health facility or through self-management techniques. When complications arise, the patient’s overall health would decline because it worsens the situation. For example, given that the patient cannot effectively take care of his health at this point, complications would make it harder to manage the disease.
Another possible adverse outcome that could occur with this patient is limited education about important care sites. While patients can access care, a problem occurs when they cannot access the right care (Nazar et al., 2016). Patients with sufficient information about vital care sites often have improved health outcomes (Nazar et al., 2016). For example, the paralyzed patient with diabetes needs education and access to information about diabetes self-management, as well as best way to take care of himself with the paralysis. Currently, the difficulties he faces accessing care indicate a high likelihood that he is not accessing important care sites (Nazar et al., 2016). If he cannot come to the hospital or move around easily, it becomes difficult to obtain the necessary information he needs to improve his health outcomes. He needs to access information such as how to manage diabetes, as well as physical therapy to improve his mobility. Limited access to vital care sites can mean that the patient may be unaware of vital information that could improve his health.
The assessment and data that would need to be monitored with this patient is cultural competence care.
