PICOT – In an acute hospital setting, how does the implementation of near-miss medication protocols and a mandatory reporting system compare to the improved reduction of actual adverse nursing events and improve overall patient safety?
In 500‐750 words (not including the title page and reference page), apply a change model to the implementation plan. Include the following:
Rogers' diffusion of innovation theory is a particularly good theoretical framework to apply to an EBP project. However, students may also choose to use change models, such as Duck's change curve model or the transtheoretical model of behavioral change. Other conceptual models presented such as a utilization model (Stetler's model) and EBP models (the Iowa model and ARCC model) can also be used as a framework for applying your evidence‐based intervention in clinical practice.
Apply one of the above models and carry your implementation through each of the stages, phases, or steps identified in the chosen model.
In addition, create a conceptual model of the project. Although you will not be submitting the conceptual model you design in Topic 5 with the narrative, the conceptual model should be placed in the appendices for the final paper.
Prepare this assignment according to the APA guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

Iowa Model.
Since the introduction of evidence-based research in the 20th Century, evidence-based practices have increased gradually in all health care settings. In the world right now, nurses are the part of a health care environment who are argued to be the main users of updated research evidence to provide the best possible care to their patients. This is due to the huge change in the notion of decision making (Mabbott, 2017). Therefore, the information that the nurses use to make solid decisions in their practices are referred to as evidence although their validity may be variable. For an operation to be conducted in an evidence-based manner, there should be a way of introducing, developing, evaluating and examining the practice. This paper seeks to use the Iowa model to offer a practical evidence of the proposed solution of near-miss events and a mandatory reporting system.
Steps of the Iowa Model.
Step 1: Selecting a topic.
In this section, the selected topic for the project will be on curbing and eliminating the issue of near-miss errors in particular situations by devising a reporting system that informs the facility’s health care practitioners of the nature of the issue prior to its actual occurrence.
Step 2: Forming a team.
The team will consist of a number of stakeholders who will contribute to the project by assisting the eight specialist staff members and the six members who will be situated in the field (Mazurek Melnyk, 2016). The team will be tasked with the responsibility of carrying out the implementation of the program by providing the required resources, managing and organizing the group, drawing up appropriate and required policies, procedures and guidelines as well as directing the nurses in the facility on the manner in which they can enforce the proposed changes.
Step 3: Retrieval of Evidence.
In this step, a group of specialists involved in this project will sit down and select the best from the numerous members of the team to aid in the actual implementation of the proposed solution to errors in hospitals. This is also the step where the topic will be selected by brainstorming a number of instances in hospitals where the same thought we have has or is currently occuring (Steelman, 2016). Finally, the team will utilize the Cinahl and Medline methods to retrieve data and resources in form of evidence using electronic methods.
Step 4: Grading of Evidence.
First of all, as stated in the previous document, this research will be quantitative as it will be based on deduction, testing of hypotheses and other objective methods of data collection with the aim of controlling the phenomena (Buckwalter et al., 2017)....
