CAPSTONE CHANGE PROJECT PLAN 

 The purpose of this assignment is to apply nursing leadership and capstone concepts learned over the course of the semester to bring about change.  The change can be actual, potential, or hypothetical.  It must be addressed within the context of a health care setting and relate management concepts.  The change should be written as though it will be implemented sometime within the next year - not a description of a change that has already been implemented.  

Ideas for the Capstone Change Project Plan include but are not limited to the following:  

  • Developing staff training or educational programs
  • Introducing a change on a unit such as one to be in compliance with The Joint Commission
  • Developing and implementing new procedures/policies
  • Developing a specific protocol
  • Developing of a parish nurse program
  • Developing of a community health fair
  • Implementing a new piece of equipment
  • Implementing a best practice or quality improvement plan
  • Creating a new leadership structure
  • Improving recruitment or retention
  • Dealing with conflict

 

The setting may be real or fictitious, but you should determine the location, size, setting, leadership models, etc.  Anticipated length of paper is 7-8 pages (not including cover page or references section).  Your paper must contain a minimum of 5 scholarly (peer reviewed) journal articles cited in the paper to support your chosen change and included in the reference list (this is in addition to any textbook citations/references). Please submit to the D2L “Assignments” area.

Description of the Health Care Setting  

¨ Describe the setting where the place will take change. Identity the context in which the change will occur.  It can be any health care setting, such as a clinic, community agency, long term care facility, hospital, etc.  Your proposed change can pertain to the system or a unit within the system, a group of workers within the system, a shift within the system, etc.

Developing a Community Health Fair
The community stakeholders involved in the community health promotion will include the various organizations of people involved in the evaluation process of specific public health programs. For example, those engaged in multiple program operations include funding agencies, management, program staff, partners, and coalition members. The stakeholders affected by the program will consist of community members, the elected officials, patients, and advocacy groups who are facing various health crises such as lack of medical insurance, drug addiction, and inadequate prenatal care. Another group of community stakeholders includes funding agencies, decision-makers of the program, the general public, or coalition members (Das et al., 2016). These diverse stakeholders will help in the advocacy and management of the program.
The community stakeholders will be helpful because they will be responsible for daily activities within the community health fair. They will also advocate for or make immediate changes to the program if needed. Also, the stakeholders will increase the credibility of the efforts of the community health fair. Besides, these stakeholders may be great financers of continuing or expanding various activities in the community fair.
For proper ethical and legal alignment of the goals and outcomes of the community health fair, the stakeholders may participate in the program evaluation process to offer helpful insights into the program. Possible legal or ethical implications include the use of unqualified screeners or organizers. The use of this group could lead to the delivery of unreliable information, which could create problems. The process could lead to low efficacy; hence the results could not be conclusive (Das et al., 2016). The patients could receive negative consequences where they may think that they do not need comprehensive medical care, but they do.
The communication strategies will involve volunteers or persons who understand the local dialect and environs of the target population. The communication structure will include the formal and informal communication structure to reach a wider audience and promote professionalism. The decisions will be made in an organized system and using the top to bottom approach. The volunteers will be instructed to initiate an interactive conversation with community members to acquire the most favorable outcome of the fair (Howell, 2018). The desired outcome is the mobilization of minority groups to learn various health messages and listen to community voices. The change will be communicated by carrying out a perception analysis of the success rate of the event. The results analysis will determine if they're in need to promote the community health fair further. The necessary leadership support includes government or private stakeholder involvement in mobilizing resources or overall health reforms for the minority groups.
