Setting and Access to Potential Subjects
This study’s sampling method is purposive sampling, which is used to select a particular set of people with specific characteristics. It shall be used because only a limited number of people have the disease and those who have access to healthcare (Etikan & Bala, 2017). In this case, patients of all ages and both sexes with Amyotrophic Lateral Sclerosis (ALS) shall be included.
ALS patients shall be identified through the University of Chicago Medicine Hospital’s system records. Consent forms shall be provided for the participants to understand the terms and conditions of the research or the participants’ guardian if they are incapacitated. Furthermore, authorization forms shall be sent to the university’s and hospital’s ethics review committees to approve the research. An authorization letter shall also be sent to the Neurology Department Head of the University of Chicago Medicine.
Timeline
The time needed from the preparation to implement the research and interpretation of the results will take around six (6) months. Specifically, the following are the estimated duration for each phase (See Appendix for the Gantt Chart):

1 Writing of authorization letters and consent forms
2 Gathering the approval from the ethics committee of the university and the hospital
3 Identifying the participants through the University of Chicago Medicine Hospital’s system
4 Gathering the approval forms of the participants

5 Writing of the questionnaires and incorporating the outcome measures needed to identify disease-related complications
6 Training of the researchers assigned for the tele-interviews and in-clinic visits

9 Dissemination of the information to the stakeholders (i.e., patients, hospital, and university)
Resources
The study needs human, fiscal, and material resources. Remarkably, human resources involve the researchers, who shall conduct the interviews and observations. The researchers are also responsible for data gathering and inputting the data through electronic resources and coordinating with the Head of the Department of Neurology to gather the data through the physician’s help. Other human resources include the participants or patients with ALS and/or their guardians or primary caregivers, who shall be contacted during each visit or follow-up if the participant is incapacitated.
