Background
Hospital-acquired infections (HAIs) are a leading cause of death. HAIs are infections that patients develop in the course of receiving treatment in a healthcare facility (Haque et al., 2018). HAI threatens patient safety and recovery. According to Haverstick et al. (2017), HAIs are mostly transmitted through contaminated hands. This occurs among both patients and healthcare workers. Thus, effective hand hygiene practices have been considered an effective way of reducing the spread of HAIs (Hillier, 2020; Yousef et al., 2020; Nyamadzawo et al., 2020; Banks & Philips, 2021).
Clinical problem statement
Poor hand hygiene among healthcare workers and patients in a hospital setting is a major cause of concern. Despite evidence showing that effective hand hygiene reduces the spread of HAIs, the levels of compliance with hand hygiene practices are quite low. According to Fish et al. (2021), there is only 40% compliance with hand hygiene practices in many healthcare settings. For example, at Veterans Affairs (VA) Medical Center, compliance with hand hygiene has declined. This is quite alarming, given that the VA was one of the early adopters of practices aimed at reducing the spread of HAIs (Vaugn et al., 2020). It is important to address this issue since VA has over 120 inpatient facilities (Vaugn et al., 2020), and poor hand hygiene threatens the safety of patients in these facilities.
Purpose of the change proposal
The idea behind the change proposal is to reduce the spread of HAIs at VA Medical Center. It will also facilitate compliance with hand hygiene practices.

Literature search strategy employed
The literature search strategy employed involved determining the keywords and key terms to be used in searching for relevant literature. The keywords and terms used were hand hygiene, hand washing, hand decontamination, hand cleaning, hand hygiene compliance, hospital-acquired infections, healthcare-acquired infections, and healthcare-associated infections. Boolean operators “OR” and “AND” were used to combine the key terms during the literature search. The search was conducted in various databases, including Google Scholar and CINAHL.
Evaluation of the literature
The first step in the evaluation of the literature was to establish the quality of the research. This was done by ensuring all articles were peer-reviewed. The credibility was determined by identifying the authors and their areas of expertise. Most authors were registered nurses with experience working in a healthcare setting. To determine the relevance of the literature materials to the capstone project, the analysis used in each article as well as the main findings, were examined. The publication date of the articles was also examined to ensure that the information in the articles was current and relevant based on the current state of healthcare settings.
Applicable change or nursing theory utilized.
The theory that was deemed most applicable for change implementation, in this case, was the Knowledge-to-Action (KTA) framework. The KTA framework is used when shifting from evidence to action by facilitating the implementation of change. It comprises two phases, namely, knowledge creation and action cycle (Grinspun & Bajnok, 2018). Knowledge creation refers to the process through which knowledge about an intervention is accumulated and synthesized. The action cycle refers to how knowledge is applied to real situations, which helps in changing the status quo. For example, the knowledge about hand hygiene has already been accumulated, but there is a need for more practical synthesis through the implementation of practice guidelines. Healthcare workers can be trained on effective hand hygiene as part of knowledge creation. Healthcare workers can then use this knowledge to enhance hand hygiene compliance as part of the action cycle.
