Ethical Dilemma in Pediatric Setting
Vaccination of Minors
Vaccination of minors is a pediatric dilemma when the legal guardians refuse to vaccinate the child, and the minor cannot decide. The dilemma arises through respecting parental informed consent and autonomy and the duty of the pediatrician to ensure public health through vaccination mandates. The dilemma is established through various positions.
In recent studies, there has been a rise in cases of preventable diseases, which has put the American Nurse Association(ANA) position of review in consideration of religious beliefs or medical contradictions (Schwartz et al., 2021). The ANA position states that all healthcare personnel must be vaccinated according to current recommendations for immunization and CDC.
Similarly, the code of ethics provides guidelines for healthcare to offer vaccination to children to safeguard community and population health. However, the law states that a parent is the sole guardian to make healthcare decisions on the child's vaccination and can opt not to vaccinate their children. Therefore, enforcing the code of conduct will directly influence the parent's decision. The dilemma remains if the healthcare provider offers the vaccine or respects the parent's wish to abstain from vaccinating their children.
Position 1
Relevance to ANA Code of Ethics. The code of ethics states that the main goal is to ensure vaccines benefit the global community, and the ethical principles of autonomy, beneficence, non-maleficence, and justice must be examined and adhered to in distribution, development, and implementation (Jalilian et al., 2023). Acknowledging individual autonomy and supporting patients' right to decide for their children. When the parent decides against vaccination, it is a dilemma for the pediatrician to execute the code of ethics.
The nurse's primary goal and commitment is to the patient, and they must follow the code of ethics to ensure absolute professionalism. The nurse's interest must align with the community's interest in promoting community health, preventing harm, and the population's well-being. The ANA code of ethics provides the framework the nurses adopt to support their role in promoting and pushing for community welfare (Grace et al., 2023). It also guides safeguarding them and avoiding collision with the dilemma of respecting parents' choices.
Position 2
Parental Autonomy and Informed Consent
Parents maintain the right to decide on their child's health, including treatment and vaccination. They have the right to be fully informed and consent before administering or consulting the child's health. The parent's autonomy is grounded in ethical principles and the principle of justice (Reiss & Karako, 2019). The principle states that the parents are the primary decision makers for children's well-being; therefore, enforcing vaccination infringes this right. The right preserves and supports parents to make informed decisions about their children.
The two positions are the main cause of the dilemma as they oppose each other. That means that when a parent chooses not to receive vaccination for the chil...
