Define the patient, family, or population health problem that will be the focus of your capstone project. Assess the problem from a leadership, collaboration, communication, change management, and policy perspective and establish your evidence and research base to plan, implement, and share findings related to your project.
Part 2
Connect with the patient, family, or group you’ll work with during your practicum. During this portion of your practicum, plan to spend at least 2 hours meeting with the patient, family, or group and, if desired, consulting with subject matter and industry experts of your choice.

Chronic kidney disease is the presence of albumin beyond 30 mg/g of creatinine and GFR (glomerular filtration rate) below 60mL/min, along with kidney function abnormalities for more than three months (Hashmi et al., 2019). ESRD (end-stage renal disease) is when the GFR goes below 15mL/min (Hashmi et al., 2019). In the neighborhood, a 65-year-old male has ESRD. Hashmi et al. (2019) reported that approximately 500,000 individuals in the United States live with ESRD. The prevalence of ESRD in elders is higher compared to younger people. People aged 65-74 years have a higher prevalence of ESRD (Ahmed & Catic, 2018). The greatest incident rate is among 75 years old individuals or older. ESRD is a significant cause of premature mortality and poor quality of life. ESRD requires aggressive monitoring for signs and early referral for renal therapy to improve the patient's quality of life (Hashmi et al., 2019)
Chronic conditions such as hypertension and type 2 diabetes mellitus, which are common in older people compared to younger individuals, contribute to the higher prevalence of ESRD in geriatrics (Ahmed & Catic, 2018). Therefore, leaders, government, and health personnel must collaborate with the community about treatment and preventive strategies for ESRD in geriatrics. Each inter-professional team member must act collaboratively, maintain updated and accurate patient records, and communicate with other team members to ensure optimal patient care and improve outcomes (Hashmi et al., 2019). The government should plan for changes and implement health policies to reduce the higher prevalence of ESRD in the geriatric population. A 2-hour meeting with a neighbor and his family revealed that he has been living with type 2 diabetes mellitus for 30 years and was diagnosed with ESRD five months ago. He has been managing his diabetes with medication and good lifestyle habits. Nevertheless, the laboratory results revealed that the man had ESRD. ESRD is relevant to nursing practice for baccalaureate-prepared nurses since they gain practical knowledge and skills for managing the condition in their entire nursing practice.
Various nursing actions are required to prevent ESRD complications and enhance renal functions. Nursing education to the patient and the family is necessary to inform them of the needed lifestyle modifications. Nurses should collaborate with nutritionists to educate patients on nutrition intake. They should monitor the patients to ensure adequate nutrient intake and restrictions of potassium, phosphorous, fluids, sodium, and proteins (Hashmi et al., 2019). During clinical hospitalization, nurses should protect the patient's arm for fistula placement and place limb restrictions to ensure blood pressure readings and venipunctures are not taken on that specific arm (Hashmi et al., 2019). Nurses treat and manage underlying causes such as proteinuria, blood pressure, and diabetes.
