The article Development of the diabetes specialist nurse team to support delivery of community-based care is written by Sian Bodman. Bodman is a Senior Nurse in Diabetes with the Aneurin Bevan University Health Board and in this article, she provides a personal account of how her leadership and vision helped to create a team of diabetes nurse specialists to help offer patient-centered care.
In her article, Bodman starts by talking about the challenges that led to her appointment and the need for change at the Aneurin Bevan University Health Board (ABUHB). She mentions normal and universal challenges in primary care including “long waits for consultant diabetologist appointments,” and that “primary care practitioners were unable to refer directly to diabetes specialist nurses (DSNs)” (Bodman, 69, 2019). She continues to reveal that primary care practitioners believed in their skills and thus wanted to cater for more complex diabetes needs but needed support. So, this presented an opportunity for ABUHB to develop a project that will help bridge the gap between secondary and primary care while enhancing patient care.
The diabetes redesign project’s sole purpose was to ensure that the issues or challenges raised could be dealt with. Bodman was appointed Senior DSN and was made in charge of the DSN team. She was tasked with recruiting a new DSN team that was to work in primary care. It was also agreed that ABUHB adopts the Portsmouth Super Six model of diabetes care which meant an integration of secondary and primary care though not entirely.
The changes that were made meant that the role of the primary care DSNs (PCDSN) had to be defined. So, the PCDSNs were expected to review patients’ health profiles as well as their test results. This team was recruited by Bodman and she also doubled as their mentor alongside other DSNs, a diabetes specialist, and consultants. The training of PCDSNs was conducted by this team of mentors and it was agreed that by year’s end the team was supposed to have finished their training.
