For this assignment, you will identify a clinical or administrative issue and propose an informatics solution. The problem you identify will be the focus of a project for assignments in weeks two through week five. Each week, you will work on an element of this project. In Week 5, you will synthesize your work into a proposal appropriate for submission to the administrators/directors of a health care organization.
This week’s assignment, Informatics Solution Proposal, you will propose an informatics or technologic solution for a problem currently found within your clinical or workplace setting. Throughout the next four weeks you will review the functionality of various products or technology, and select the one product that best meets the needs of a health care setting.
Identify an issue/problem in a health care workplace that could be resolved by implementing an informatics or technological solution. The issue/problem can be an administrative or a clinical issue.
Draft a 350-word letter or memo to a nursing administrator. In the memo, identify the clinical or administrative problem, address why the problem is an important concern, and, request permission to propose a solution. Support your letter with at least three peer-reviewed resources.
Format the letter according to APA guidelines for formal business correspondence.
Provide an APA-formatted reference page for all resources used. There are three required references for this assignment.

One of the major issues that negatively impact patient safety is a communication breakdown, especially during patient handoff. A breakdown in communication between one caregiver to another can cripple care continuity and infringe on patient safety. When these breakdowns occur in handoffs, the repercussions could be dire, with many errors expected (Shahid & Thomas, 2018). Other issues that could be reported include treatment delays, care omissions, major harm, readmissions, and even inapt treatment.
When a handoff is mishandled, the workflow is also affected. Nurses and physicians may have to do more for a patient because they could be undoing the care already provided to a patient at any one instance. With issues such as care omissions, inapt treatment, and readmissions happening, healthcare professionals will indeed find themselves with more on their hands than is usual. The sad thing about the cause of all these issues is that the problem is preventable.
Aside from impacting workflow, communication breakdown during handoffs can endanger the lives of patients. Raeisi, Rarani, and Soltani (2019) note that when communication breaks down during a handoff, safety and quality of care are compromised.
