I agree with you Kacye that lack of communication one of the team failures. In a team, some members may feel like their feedback is not appreciated, making them to avoid communicating (Kreitner & Kinicki, 2010). A conscious culture ensures it takes into account all perspectives of stakeholder and to achieve this; I would allow each employee to be heard to create a smooth level of communication.
Damien
Lack of a clear team dynamic is a prominent reason team end up failing. As you suggest, Damien, conscious culture guides a team in acting and performing purposely towards success. However, when a team fails to share goals, vision, and how work should progress a team breakdown. To ensure the team is successful, I would adopt a conscious culture where every role and action is defined, to ensure all activities undertaken are intentional (Anderson, 2015, Jun 1).

I agree with you that the lack of effective leadership can lead to team failure. When leaders in a team fail to promote the productivity and the morale of the team members, disappointment is imminent (Kreitner & Kinicki, 2010). Every leader needs to be competent, motivating, and open to listening to their employees (Anderson, 2015, Jun 1).
