Based on your community assessment, including your windshield survey and the assessment information from your discussions, identify one aggregate or vulnerable population in your community that faces a community health problem. See units five (5) and six (6) in your Nies and McEwen text for examples of vulnerable populations and aggregates. Your community must be the area where you live or work.
A. Introduction (one paragraph): The introduction should be interesting and capture the reader's attention.
- Provide a brief description of your community including the city name and state and key features.
- Briefly introduce one vulnerable population that you identified in your assessment.
- Discuss the purpose of your paper. The purpose statement of the paper should relate to assessing this vulnerable population in your community.
B. Vulnerable Population Overview (one to two paragraphs):
- Describe one vulnerable group in your community in greater detail.
- Include your observations about this group from the windshield survey.
- Discuss events or trends that have affected this vulnerable group such as economic conditions, trends in health related behaviors or disease rates, etc.
C. Strengths, Risk Factors, and/or Barriers (two to three paragraphs):
- Discuss the strengths, risk factors, and/or barriers that impact this vulnerable population's health.
- Include support from at least one scholarly peer-reviewed journal article in this discussion. Go to the Chamberlain library at http:/library.chamberlain.edu. Search peer reviewed journals to find a scholarly article that discuss the strengths, risk factors, and/or barriers faced by this vulnerable group. Summarize what you learned from this article and cite the source in the text and reference page (Author, year). For this section, you must choose at least one scholarly article from a peer-reviewed journal. The article must be less than 10 years old (less than 5 years is best). You may include and cite your textbook or other scholarly sources as well, but they do not count as the scholarly journal article.
- Please do not include more than two short quotations, at the most. You will need to summarize and analyze the information from the article in your own words.
D. Community Resources (one to two paragraphs): Search your local public health department website, the internet, the local newspaper and/or telephone book for health resources available in your community for this vulnerable group.
- Briefly describe the resources in your community that are available for this group.
- Are these resources adequate? Discuss gaps in services that you identified.
E. Community Health Problem Diagnosis (one to two paragraphs):
- Based on your assessment and the Healthy People 2020 objectives, identify and discuss one priority community health problem that a community health nurse could positively impact. Examples include an increase in teen pregnancies, prevalence of cancer, high incidence of tobacco use, above average infant mortality etc. The problem must relate to the vulnerable population you identified.
- Go to the Healthy People topic areas at http://www(dot)healthypeople(dot)gov/2020/topics-objectives. Find a topic area that relates to your vulnerable population. Click on the topic area and then click on the green “Objectives” tab. Find and state the Healthy People 2020 specific numbered objective (not just a goal) that describes your problem (ex. for teen pregnancy, the topic area Family Planning is relevant, click the green objectives tab and you might choose FP-8.1 “Reduce pregnancies among adolescent females aged 15 to 17 years” (US Department of Health and Human Services: Healthy People 2020, 2016, Objectives: Family Planning, para 8).
F. Summary (one paragraph):
- Summarize important points from the body of your paper including the vulnerable group you identified and key factors that you discussed above that affect the health of this group
- Include a statement about the priority problem you identified.
- End with a concluding statement.
G. Reference Page: The Reference Page should start on a new page (insert a page break). All references should be cited within the body of the paper as (Author, year) and the full reference should be included in APA format on the reference page. 1. Introduction of Community (20 points)
Identify the city and state of your community and briefly describe the community you will be using for this assignment. It should be the area where you live or the area surrounding your work setting, but must include a residential area. Demographic data are not needed. The community that I will be using is Miami Gardens, Florida, where I live for the past twelve years. The boundaries that follow are country line Road, to the North 151st to the south, North Miami Avenue/NE2nd Avenue to the East, NW47th avenue and NW57th avenue to the West.
2. Windshield Survey (75 points)
a. Vitality: Use a majority of the questions from Box 6-2 in your text to describe your observations about the community vitality. Miami Gardens is a vibrant and a diverse city with a strong sense of community ownership, abundant employment opportunities and cultural and leisure activity, especially for our senior residents Parks and playground are in abundance. They are very convenient and in walking distance. Children often visit with their parents in the afternoon, on weekends or whenever is convenient. While the children are playing, parents are often seen studying, reading, exercising, or playing with their kids.
Miami Gardens is convenient. We have several shopping center with a wide variety of stores in walking distance from the residential home.
b. Indicators of social and economic conditions: Use a majority of the questions from Box 6-2 in your text to describe your observations about the social and economic conditions. Miami-Dade County has experienced a steady and rapid population growth, particularly because it is led by a diversified group of sectors. Each of the following sectors account for more than 10 percent of Miami-Dade employment. For Miami-Dade County over one-half of its residents were foreign born. Current income figures for the County are quite low by national standards. Both median household and median family income are approximately 80 percent of the corresponding figures for the nation. The current low income levels generate high poverty levels in Miami-Dade County. The two factors most closely associated with these high poverty rates are low levels of educational attainment and family structure.
c. Health Resources: Use a majority of the questions from Box 6-2 in your text to describe your observations about the health resources. From my observation Miami-Dade County has a lot of health resources. They provide educational and clinical services to the community, they provide free vaccine for all children 0-18 years, free immunization services to protect resident's especially pregnant mother from vaccine preventable diseases, free clinics for everyone regardless of your financial status. These services are available to all residents and visitors. Citizenship is not a requirement. They don't denies services base on a person's race, color, religion, sexual orientation or national origin.
d. Environmental conditions related to health: Use a majority of the questions from Box 6-2 in your text to describe your observations about the environmental conditions. Travelling throughout the city of Miami Gardens I have found that, depending on the area, some areas do have a tendency to flood during heavy rain that increase the pile of standing water, which increases the risk of the Zika virus. The way this community is designed, there are lots of fast food store next to each other, but limited access to paths for biking and walking. The usage of so many motor vehicle surely impacts the environment in a negative way, with so many emission this can affect the respiratory system and cause health problem to be increased.
e. Social functioning: Use a majority of the questions from Box 6-2 in your text to describe your observations about the social functioning. There were a lot of social functioning among the teenagers, middle adult's and older adults People participate in a variety of different things such as, party, cooking out together with family or just gathering in public. My community has special location where people worship, meeting for business, fellowship, celebration, play games, hold support groups, sing, participate in programs that go outside the community and volunteer to serve cook and uncooked meal and go on a field trip. We create a community in which we can grow, bond, and strengthen our connection.
f. Attitude toward healthcare: Use a majority of the questions from Box 6-2 in your text to describe your observations about the attitudes toward healthcare. 70% of the Miami Dade Country are African Americans, they have distrust in health care, especially the older male that I have spoken with they claim that is because of the mistreatment at the hands of the medical profession, the Tuskegee syphilis experiment, I found out that the Caucasian patients, likely to differ for African Americans. They will go for treatment regardless of their status or payment method.
3. Vulnerable population (20 points) Describe a vulnerable population or aggregate that you identified during your survey. What did you observe about this group such as their appearance and actions? The vulnerable population I identify in my survey is the amount of homeless people on the street asking for food or selling juice or water in the hot sun on a daily basis.
4. Conclusion (20 points)
Provide a summary of your findings and your conclusion. What potential problems did you identify? Reducing the use of cars and shifting to more walking and biking not only positively impact the environment, but also improve health through increased physical activity, avoiding emissions.
The surrounding environment affects our health and physical activity. Communities, homes and workplaces can all influence personal health. Because of this influence, it is important to create communities that make it easy to engage in physical activity and to eat a healthy diet.
Access to healthier food and making healthier food choices, including eating less processed foods and eating more locally produced fresh foods lowers risk of obesity and other health conditions.

The relationship between nursing interventions, health risks and resource availability influence health decisions that affect the homeless people as a vulnerable population. The homeless people in Miami Gardens, Florida are a vulnerable population who face difficulties accessing health especially the mental health services. Knowledge and better understanding of the health problems that affect the homeless population, would improve health outcomes (Nies & McEwen, 2015). Homelessness has been viewed as a socio-political issue, but the population is at greater risk of health problems. Hence, awareness of the healing needs of the homeless population is crucial to improving the delivery of quality health care. This paper focuses on the homeless people in Miami Gardens, Florida as a vulnerable population who are affected by mental health issues.
Vulnerable Population Overview
The homeless people are vulnerable as they lack health insurance, meaning that they cannot seek health care treatment and medications for their conditions. In some cases, poor health causes homelessness, and this is particular for those already facing financial difficulties, any savings are quickly used up for medical purposes without improving health outcomes (Nies & McEwen, 2015). Among the homeless people, the African- American group is skeptical of medical intervention compared to Caucasians even under similar circumstances. Not all adults are eligible for social services, and even when they are poor and uninsured, while loss in income job losses and having no income can increase the risk of homelessness.
In the Miami Gardens, Florida the homeless tended to have more health problems because of exposure to violence, communicable diseases and malnutrition. In the case for those take in medications there are no safe places to store the drugs, making it difficult to follow up on patient medical adherence. Additionally, the problems plaguing the homeless population also occur together with other conditions including substance, psychiatric and social problems (Nies, & McEwen, 2015). Despite facing many challenges and dangers living on the streets, the homeless population in Miami Gardens, Florida also has resilience. This is especially those who know where to find resources and have adapted to the social structures. Even as the behaviors are not necessarily prosocial, the ability of homeless people to connect with others in similar situations can often allow them to use peer networking to cope with their situation (Pratt, 2012). Homeless people are at high risk of violence, exploitation and sexual abuse, and through networking they can identify resources available and avoid people or locations that are dangerous.
