Assignment BACKGROUND INFORMATION
The client is a 26-year-old woman of Korean descent who presents to her first appointment following a 21-day hospitalization for onset of acute mania. She was diagnosed with bipolar I disorder. Upon arrival in your office, she is quite “busy,” playing with things on your desk and shifting from side to side in her chair. She informs you that “they said I was bipolar, I don’t believe that, do you? I just like to talk, and dance, and sing. Did I tell you that I liked to cook?” She weights 110 lbs. and is 5’ 5”
SUBJECTIVE
Patient reports “fantastic” mood. Reports that she sleeps about 5 hours/night to which she adds “I hate sleep, it’s no fun.”
You reviewed her hospital records and find that she has been medically worked up by a physician who reported her to be in overall good health. Lab studies were all within normal limits. You find that the patient had genetic testing in the hospital (specifically GeneSight testing) as none of the medications that they were treating her with seemed to work. Genetic testing reveals that she is positive for CYP2D6*10 allele. Patient confesses that she stopped taking her lithium (which was prescribed in the hospital) since she was discharged two weeks ago.
MENTAL STATUS EXAM
The patient is alert, oriented to person, place, time, and event. She is dressed quite oddly- wearing what appears to be an evening gown to her appointment. Speech is rapid, pressured, tangential. Self-reported mood is euthymic. Affect broad. Patient denies visual or auditory hallucinations, no overt delusional or paranoid thought processes readily apparent. Judgment is grossly intact, but insight is clearly impaired. She is currently denying suicidal or homicidal ideation.
The Young Mania Rating Scale (YMRS) score is 22

Introduction
Bipolar disorder, also known as manic depression, causes a person to experience frequent depression and euphoria. The condition, which is a form of mental illness, impairs the individual disposition and social settings due to abnormal activities. Bipolar disorder is characterized by symptoms such as mood swings, depression, and mania. The disorder becomes too complex, making it hard to diagnose other disorders such as schizophrenia since it has the same symptoms (Carvalho et al. 2020). The disorder always results in a series of episodes which makes the patient experience delusions and hallucinations. A chemical imbalance also characterizes the disorder, thus impairing the individual brain, which triggers mood swings. This paper will examine a 26-year-old woman diagnosed with bipolar disorder, and an attempt to make clinical decisions will be analyzed to optimize her treatment.

On medical review, the woman was diagnosed with bipolar one disorder. The physician reported that the patient is in good health with the lab limits operating within the normal limits (Grande et al., 2016). The patient had GeneSight testing, with none of the medication failed to work for her. The genetic testing is a clear revelation that the patient is positive for the CYP2D6*10 allele. Furthermore, the patient confessed that she stopped taking her lithium, a prescription from the hospital since she was discharged a fortnight. Therefore, after receiving all this information, an ultimate decision ought to be made between Begin Lithium 300 mg orally BID, Begin Risperdal 1 mg orally BID, Begin Seroquel XR 100 mg orally at HS (Berk et al. 2017).
Decision One 
I chose to begin Lithium 300mg orally BID. This is the ultimate decision since it is always designed as a startup medication for patients with bipolar disorder. The rationale for this prescription is that the patient ought to be induced to small amounts of the dosage to monitor the medication's progress (López-Muñoz et al., 2018). Therefore, the patient will be taking Lithium 300mg daily will be a beginning and a way of evaluating the patient's progress. At this point, the physicians can determine the drug's reaction on the patient and its outcome, thus providing an opportunity for them to pick an alternative option in case the decision failed to materialize.
The results that I expected, in this case, are an improvement when compared to the initial recording of the patient since he started medication. I also expect the patient to experience a state of rest and a drastic reduction of the high levels' activeness (Grande et al., 2016). I decided over the other two since this decision is always the start, and the problem can be solved at this stage.
