This assignment requires you to interview one person and requires an analysis of your interview experience.
Part I: Interview
Select a patient, a family member, or a friend to interview. Be sure to focus on the interviewee's experience as a patient, regardless of whom you choose to interview.
Review The Joint Commission resource found in topic materials, which provides some guidelines for creating spiritual assessment tools for evaluating the spiritual needs of patients. Using this resource and any other guidelines/examples that you can find, create your own tool for assessing the spiritual needs of patients.
Your spiritual needs assessment survey must include a minimum of five questions that can be answered during the interview. During the interview, document the interviewee's responses.
The transcript should include the questions asked and the answers provided. Be sure to record the responses during the interview by taking detailed notes. Omit specific names and other personal information through which the interviewee can be determined.
Part II: Analysis
Write a 500-750 word analysis of your interview experience. Be sure to exclude specific names and other personal information from the interview. Instead, provide demographics such as sex, age, ethnicity, and religion. Include the following in your response:
What went well?
Were there any barriers or challenges that inhibited your ability to complete the assessment tool? How would you address these in the future or change your assessment to better address these challenges?
How can this tool assist you in providing appropriate interventions to meet the needs of your patient?
Did you discover that illness and stress amplified the spiritual concern and needs of your interviewee? Explain your answer with examples.
Submit both the transcript of the interview and the analysis of your results. This should be submitted as one document. The interview transcript does not figure into the word count.

For a long period of time, the roles of the spiritual and the physical healer have both been very divergent from each other CITATION Shi15 \l 1033 (Shields, Kestenbaum, & Dunn, 2015). With one focusing on spirit and the mind, while the other on the biophysical component of the body, both practices rarely find the need for the help of the other. However, in the recent decades, a more holistic trend that employs both spiritual and physical healing are now being used "within health-care systems" CITATION Rum13 \l 1033 (Rumbold, 2013). According to Rumbold (2013), "these changes [to the role of spiritual assessment] is indicative both of changing attitudes to spirituality and religion in society and changed patterns of care within health care systems". Perhaps, one of the best examples that portray this ongoing trend is the increase in hospitals that are requiring board-certified chaplains that can conduct spiritual assessments CITATION Shi15 \l 1033 (Shields, Kestenbaum, & Dunn, 2015). More specifically, the importance of spiritual treatments and healing are widely recognized to be critical to hospital treatments since it provides palliative and "good end-of-life" care CITATION Bla15 \l 1033 (Blaber, Jones, & Willis, 2015). Another importance of spiritual treatment is its "short, versatile, and focused approach" that gives chaplains - who doesn’t have the luxury of time in acute settings - enough time to provide palliative help to its patient.
In line with all of this, I conducted an interview and designed the paper to provide a good template for conducting spiritual assessments. More specifically, the interview and this paper were designed using FACT as a template to maximize the help that we need to a patient with an acute illness.
Spiritual Assessment Tool
During the interview, I chose to use FACT as the template for my spiritual assessment tool. FACT stands for; Faith of beliefs, Active/available/accessible/applicable, Coping/conflicts, and Treatment plan. Unlike any other templates - such as FAITH, MEMO, FICA, etc. - FACT focuses on providing a treatment plan after assessing the spiritual needs of the individual. As LaRocca-Pitts (2012) [Fact is] …"A spiritual assessment is an in-depth look at the patient’s spiritual makeup with the goal of identifying potential areas of spiritual concern and determining an appropriate treatment plan."
Interview
Before I started with the assessment, I asked my friend if I can do the interview with his uncle that has Stage-4 Acute Lymphocytic Leukemia (aka ALL) - a type of cancer that starts in the bone marrow, spreads to the whole body, and affects the bone marrow and other major organs such as the CNS, Lymph nodes, liver, etc. His uncle is 43 years old, High School science teacher, born and raised catholic, and half-Spanish (just like his dad). Fortunately, after explaining and giving proper disclosure, I was able to get the approval of both my friend and his uncle’s (the patient). The interview goes as follows:
