Risk Management Plan: Medication Errors
Selected Risk Management Plan
The plan involves minimizing medication errors in healthcare facilities. The reason for selecting the risk management plan that seeks to minimize medication errors is to prevent the adverse effects associated with them. Each year, about 9,000 individuals die in the U.S. alone due to medication errors (Bhimji & Scherbak, 2018). Additionally, other patients experience adverse reaction or complications due to medication. When medication errors occur, they are costly to the healthcare facilities and the patients. Furthermore, when medication errors occur, they cause psychological and physical pain to the patients. Finally, medication errors lead to decreased patients’ satisfaction and increasing mistrust in the healthcare system. Given the adverse effects of medication errors on both the patients and the healthcare facilities, it is critical to have a risk management plan to mitigate this problem.
Risk Management Program in a Typical Organization
In a typical organization, the risk management plan would be more reactive. The plan would address medication errors once they occur as opposed to putting measures to ensure that these errors do not happen in the first place. For instance, the plan would emphasize the investigation of incidences once they occur. In this case, the risk management plan would provide how to undertake initial and ongoing investigations and interviews to ascertain the cause of a medication error.
Additionally, the plan would include the measures taken against healthcare professionals for medication errors. The plan would include the disciplinary measures a healthcare facility would undertake to the healthcare professionals to deter them from making errors. In such a case, healthcare professionals would be unwilling to inform the relevant authorities in case medication errors occur (Abdel-Latif, 2016). A reactive approach to dealing with medication errors is not the best approach to addressing the issue.
Risk Management Program in the Exemplar
There is a need to have a proactive approach to addressing medication errors. The first component of the plan is incident reporting (Kavaler & Spiegel, 2003). The program entails a reporting system that identifies, reports and tracts medication errors. It is meant to minimize preventable medication errors and hence reduce the financial burden of claims. The staff responsible for risk management should track the data and present the findings to the performance department for follow-up action.
The second component is the educational activities. The manager is supposed to facilitate orientation activities new members. An educational program is required to create awareness of risk and methods of preventing the occurrence of risks. Training programs to reduce medication errors should be done constantly to assist healthcare professionals to identify better ways of preventing common errors (Weant, Bailey& Baker, 2014). Further, the training programs would enable the healthcare staff to identify effective ways of dealing with errors once they occur. The training will assist in ongoing monitori...
