Tube and J- tube care      Placement of naso/orogastric tube Assisted ventilator dependent patients ages from months to 12years of age  Supervised RNs, LPNs, and Personal Attendants with and without delegated tasks. Work directly with patients and their families. Serves as the primary point of contact between the patient and other health care members in regards to changes in client care and continued need of services. Performs patient evaluations, including monitoring and tracking of vital signs; perform procedures such as IV Medication administration, phlebotomy, medication/disease process teaching. Develop day to day nursing care plan and discharge teaching. Coordinate with physicians and other healthcare providers. Ensure the home is maintained in compliance with all state rules and regulations. Train Home Health Aides in the care of a patients Link and educate clients to community resources and other Direct Care Agency providers for better decision-making.
