Public health nurse or community health nurse such as Emily plays a critical role in facilitating health care delivery to an underserved, vulnerable population. There are substantial health disparities and risk factors that expose particular groups to increase exposure to dis4eases marked by more severe morbidity, burden, and mortality in the U.S. For vulnerable populations, their health and healthcare concerns are aggravated by socio-economic factors such as disability, low-income or poverty, some geographical regions where they are grossly underfunded, very young children and seniors. This paper will focus on seniors who are at more exposed risk to more health adversities.
