Medication errors majorly occur in hospitals. According to Tariq & Scherbak (2018), the errors occur with the medication systems in hospitals. Health professionals can make mistakes that have critical impacts on the patient. For instance, medication errors can occur during drug dispensation or prescription (Alomari et al., 2018). Hospital is the major setting for the problem since there is close interaction between the patients and the health professionals. Description
Medication error is a critical problem since it leads to mortality. The common medication errors include IV and Zinc overdose and poor coordination between the prescribers and the pharmacist. Medication error is also critical since some drugs have reversible effects, such as fluoxetine. Consequently, it is the key responsibility of the health professionals to work together and collaborate to ensure high-quality care and minimize cases of medication errors.
Effect of the Problem
Medication errors have different effects on patients and their families, ranging from simple complications to morality issues. Besides, the loss of family partners could lead to mental effects such as anxiety and depression.
