During the assessment, it became evident that a culture of misunderstanding and assumption had become prominent in the organization, especially in the surgery department. Nurses were operating on the assumption that they know everything they need to know about a patient after simply reading the patient's chart. They, therefore, failed to develop effective written handoff reports. This resulted in misinformation and duplication of activities in the perioperative setting. These findings pointed to ineffective nurse handoff communication. The proposed change to address this issue has four key objectives:
1 To improve nurse-to-nurse handoff communication in a perioperative setting
2 To minimize the risk of medical error arising from incomplete/ incorrect information
3 To increase nurse engagement in the entire handoff process
4 To promote positive patient outcomes
Rationale for Each Objective
1 Nurse handoff communication facilitates the transfer of patient care responsibility and patient medical information (Barbeito et al., 2018). A standardized protocol would ensure that all the necessary information and patient care responsibilities are transferred effectively.
