The symptoms provided are a clear indication of the fact that she is suffering from microcytic and hypochromic anemia, due to lack of iron and folate. Also, she is also B12-deficient. This kind of anemia often causes the inability of the body to secrete a factor that would help in the absorption of vitamin B12 as in the case of pernicious anemia. This vitamin is crucial in DNA synthesis as well as maturation of the nucleus of erythrocytes. Pernicious anemia is the most common form of macrocytic anemia.
She is a young, energetic and hence healthy individual. As a result, the other types of anemia can be ruled out using elimination based on the characteristics of their manifestation. Her menstrual experience is a strong pointer to her condition. During menstruation, there is a loss of vital vitamins as well as minerals, most likely B12 too. In the blood loss also, she might also lose substantial amounts of iron and folate. This is the reason for the severe anemia she suffers during her menstrual cycle.
The occurrence of menorrhagia and dysmenorrhea is testament enough of the massive amount of blood that she losses during her cycle. It is from menorrhagia that she suffers symptoms such as unusually low energy levels, weakness, and general fatigue during and after her menstrual cycle (Porth & Porth, 2011). This is caused by the fact that the body has lost essential vitamins and minerals, and it takes time to recover. Her body takes a considerable amount of time to replace the lost red blood cells. Considering that she has been experiencing this over a period spanning twelve years indicates a severe case of anemia.
