Case Assignment

For the Module 1 Case Assignment, review 2 cases from the AMA Journal of Ethics (http://journalofethics.ama-assn.org/site/cases.html).

In a 2- to 3-page paper complete the following:

  1. Identify each case that you have selected.
  2. Explain in detail the ethical issue of each case.
  3. Identify the ethical principle(s) that are relevant in the cases from the 4 basic ethical principles (justice, autonomy, non-maleficence, beneficence).
  4. Explain from a healthcare professional perspective what you would have done differently in the case, or if you agree with how it was handled, explain your reasoning. This should not be opinion but a supported ethical analysis.

You are to support your analysis and views with at least 3 scholarly references (e.g., peer-reviewed journals).

Case 1
The selected case study involves Ms. K suffering from sepsis and Dr. B, who proposes video calling her family members at specific times during her hospitalization. The healthcare provider feels that showcasing particular experiences and situations of the patient to the family would dispel their home of seeing her as a fighter and assist them in making informed decisions about the patient’s care.
The different propositions suggested by Dr. B present an ethical issue in this case due to the manipulative nature of video calling the patient’s family to create a premeditated perspective (Kolbe et al., 2022). The physician’s argument about video calling the family when the patient is distressed and in uncomfortable sight and never when she is calm reveals that the primary intention is to send a manipulative perception that the patient is constantly in agony and hopeless situation (Kolbe et al., 2022). Rather than suggesting maintaining family-patient contact through video calls, the current scenario represents an active attempt to influence decision-making through selective framing of the patient’s situation. As a result, this aspect illustrates a violation of ethical care principles.
This situation invokes the value of the healthcare providers respecting the principle of autonomy during care provision. According to Molina-Mula and Gallo-Estrada (2020), physicians should remain patient-centered by ensuring they refrain from any actions that coerce or trigger judgment toward their patients. In this context, Dr. B seeks to manipulate Ms. K’s surrogate to make a particular decision through the video calls. As a result, this approach violates the principle of autonomy.
As a healthcare professional, I object to the approach depicted in this case. It would have been progressive to include the video calls but ensure the timing of the sessions remained random to create a holistic picture of the patients’ experiences. The arguments by Molina-Mula & Gallo-Estrada (2020) indicate that creating a healthy surrogate-physician relationship would improve decision-making. This approach would offer the surrogates a chance to make their unmanipulated observations.
