Scripted Dialogue Portion

Utilizing the information learned from the home visit, health histories, and discharge orders, write a scripted dialogue in which you provide Sallie Mae with education that describes her problems and the interventions identified to improve her condition. Consider Sallie Mae's physiological, psychosocial, educational, and spiritual needs when developing your dialogue.

Your dialogue should resemble a script. The following is an example of a few sentences from a scripted dialogue:

82-year-old, Mrs. Sallie Mae Fisher was discharged on Saturday after being treated for chronic congestive heart failure, chest pains, arterial fibrillation, hypertension, and dyspnea for three days. For the last six months, she had been admitted four times. In addition, Mrs. Fisher states that she is a widow, having lost her husband last year. She feels hopeless and lonely most of the time (Grand Canyon University, 2014).
She has one daughter Thelma Jean living nearby, but offers minimal support because she works full time and also takes care of her family. The following are the four main problems faced by Mrs. Fisher: having limited knowledge about her medical condition especially how to manage cognitive heart failure (CHF), not adhering to the treatment as directed by the doctor, being overwhelmed by her emotions, and being at risk of falling because of the nature her home environment (Grand Canyon University, 2014).

Owing to her limited knowledge, Mrs. Fisher cannot understand the names of all the medication and the dosage she needs to take. She further ignores the doctor's directive to have an oxygen tank at home in cases of emergency. Having been hospitalized several times is an indication that she did not follow the doctor's advice (Grand Canyon University, 2014).
Interventions/Rational
Mrs. Fisher needs to be educated on the effects of her medication; this means she needs to understand some of the consequences of not adhering to the treatment. Proper management of her condition means she needs to monitor her fluid intake and manage her weight properly to avoid gaining more weight beyond 5 Ibs in a week. She needs to take note of the common signs of peripheria edema and increased dyspnea. In her condition, adequate rest and eating healthy food will improve her condition (Greenawalt, 2009).
