Introduction
Before planning care or providing treatment to the patient, it is important to have the case history in hands as it allows doctors and nurses to make the right decision and enables the patient to receive the best treatment (El-Hefnawy, 2017). During this process, underlying or previous conditions are taken into consideration. There is a strong need for healthcare experts to document information about the patient as soon as possible, and they also need to mention whether the patient requires immediate assistance or regular treatment. Another idea is to describe which medical equipment or device needs to be used while treating the patient. Doctors and nurses should discuss the current health status, the vital conditions related to the illness, childhood diseases (if any), surgical history as well as previous hospitalization. All over the United States, medical experts are encouraged to take both opportunities and challenges into consideration while planning care for their patients. This way, they can help strengthen the whole healthcare system. They may also have to work in collaboration with policymakers in order to develop and provide culturally competent services. In this paper, I will talk about the major barriers to collecting health history so that medical experts can get to know these things and can ensure the delivery of competent services.
