The risks of revealing medical errors include significant psychological consequences, including guilt, depression, anger, inadequacy, and suicidal ideations due to perceived or real mistakes. The is also the risk of possible legal action against the medical practitioner. This could result in loss of clinical confidence, which could undermine the capacity to practice. An open culture implies failure and public trust breach. The fear of being punished makes a medical professional increasingly reluctant to reveal mistakes. Revealing the medical error could also generate shame and blame from other healthcare providers and disciplinary action from their professional organizations. The open culture could communicate that the physician is to blame for the error, resulting in job loss. Therefore, revealing the medical error could also tarnish the organization’s or healthcare professional’s reputation, particularly due to the undesirable connotation of the term “mistake” (Rodziewicz, Houseman, & Hipskind, 2021).
