Think about what you have learned with regard to the five critical steps of evidence-based practice. In a formal paper of 500-750 words, address the following:
Part 1: Defining the Problem
Identify and describe one topic that may resolve a patient-care-quality problem or issue.
Draft a possible problem statement.
Describe a problem. Explain why it is a problem, and why it is significant to your discipline. Use the literature you gathered as support for why this is a problem in relation to your practice. The problem should focus on the resolution of an issue significant to improving patient care.
Draft a purpose statement in relation to your problem statement that states what you hope to accomplish if you implemented this project.
Part 2: Defining a Searchable, Answerable Question
From what you wrote about your problem and purpose, develop searchable questions using the PICOT format that will be the basis for your implementation plan. Word count is not relative in this section. Include evidence-based resources.
General Requirements:
Prepare this assignment according to the guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

Establishment of a Patient Information Management System
This paper will be aimed at providing a solution to the patient-care-quality problem of misdiagnosis. There has been rising trend in the number of cases and claims against hospitals for misdiagnosis of patients and in order to solve this there is need to establish a sophisticated patient information management system or database. The system should be software engineered program which will ensure accuracy, credibility and reduce the risk of information breach as well as help to effectively capture, analyze, categorize and tabulate patient’s information. For every patient that will walk into a hospital, their data should be recorded and information about their diagnosis, history of infection, drugs administration, appointments and more filled into the system. Newman, 2009). The system should be accessible to every concerned party that is the patient and the attending professionals.
In addition, the system should have a section for queries where the patient is free to ask questions about their health, the diagnosis received or any information that they would require. Moreover, the platform should also allow for staff consultation between caregivers to ensure that every decision made on behalf of the patients health is backed up with concrete support from not only one professional opinion but also from others. This would rule out the chances of errors in diagnosis. Also, since following up on doctors work or appraising their performance is usually hectic when done manually, the system will ease the process since every patients progress is recorded alongside the attending doctor. Therefore, cases of incompetence, intentional errors and negligence among the staff would be easily followed up and appropriate actions taken.
