Decision #1: Differential Diagnosis
Stefanie meets the criteria for cyclothymic disorder. She meets the symptoms of hypomania including increased need to perform goal-oriented activities and decreased need for sleep (American, 20130. She sleeps for about 3 hours a night but does not feel fatigued the next day. Another hypomanic symptom is decreased ability to concentrate. Stefanie also meets the criteria for depressive symptoms. She claims that when her energy levels end, she is left feeling sad. Stefanie reports feeling depressed and empty. Stefanie’s hypomania symptoms have persisted for more than 2 years and depressive symptoms but does not meet the criteria for major depressive symptoms (American, 2013). The decision I expected in Decision #1 is Bipolar II disorder. However, Stefanie does not meet criteria for abnormally elevated mood lasting for 4 consecutive days and present nearly each day. Additionally, she only meets one symptom of mood disturbance instead of three of more.
Decision #2: Treatment Plan for Psychotherapy
Arranging to see Stefanie every 3 weeks would be an effective psychotherapy. Routine follow-up every 3 months would ensure that the client is moving in the right direction with the treatment. For example, ensuring that Stefanie is taking medications as prescribed and changes applied in case of adverse side effects. Jackson et al. (2015) notes that follow-up on patients remains vital in increasing positive outcomes while also promoting safety and minimizing liability concerns. The physician can make sure the patient is faring well and satisfied with the treatment by following up (Jackson et al., 2015).
