OVERVIEW

In most cases, nurses only stick to the given guidelines, and there is need to convince them to go the extra mile in taking care of bedridden patients. This will involve showing them how tolerance with the patients is a part of the nursing integrity to which they ascribe to from the start.
Description of current problem
The prevention of pressure ulcers is given priority across all care settings. With the emerging Centers for Medicare and Medicaid stipulations concerning pressure ulcers now in force, prevention is more important than at any other time. However, it is impossible for nurses to make any important progress in preventing pressure ulcers in the absence of pressure ulcer risk assessment, prevention planning, prevention intervention, and outcomes assessment as separate and isolated activities. The most unfortunate thing is that conflicting forces in healthcare facilities, as well as disconnected practice patterns, can prevent efficient and effective implementation of the nursing process to pressure ulcer prevention. In most cases, nurses are supposed to complete and record an all-inclusive assessment, which includes pressure ulcer risk assessments, of all the patients being cared for by the nurse within thirty minutes of reporting to their workstation.
