Comparison of Hypertension Management Recommendations
Introduction
The purpose of the paper is to determine and enhance the understanding of the current recommendations for hypertension management with a focus on recent hypertension-and disease-specific guidelines. The post will focus on the current JNC8 and ACC/AHA guidelines for hypertension through a contrast and comparison process based on thresholds for initiating treatment, treatment goals, recommended medications for the African American population, and the reasons why one guideline could be used over the other.
JNC8 Guidelines
The Eighth Joint National Committee (JNC 8) recently provided evidence-based guidelines and recommendations on medications, treatment thresholds, and treatment goals in adults' hypertension management. According to the guidelines, the critical factor that determines the initiation of pharmacologic treatment is the patient's age. For patients older than 60, treatment begins when systolic pressure is 150 mm Hg or a diastolic pressure of 90 mm Hg or higher. The treatment goal is to keep the pressure below 150- or 90-mm Hg respectively for systolic and diastolic pressure (Mahdavi et al., 2020). For patients younger than 60 years, a systolic pressure higher than 140 mm Hg or a diastolic pressure of 90 mm Hg or higher should trigger treatment. The treatment goal is to ensure that the pressures are equal to less than the threshold that triggers treatment.
Under these guidelines, the recommended initial treatment for hypertension in the African American population should include a thiazide diuretic or calcium channel blocker. The treatment goal is not achieved within one month; the initial medication dosage should be increased. Second medications such as ACE inhibitors or ARB can be added. However, caregivers should not combine ARB with an ACE inhibitor (Shrout, Rudy, & Piascik, 2017). If the adjustments do not attain the required goals, the third medication from antihypertensive drug classes (such as aldosterone antagonists) should be added.
