The Capstone Project is designed to be completed in sections. This is part three of the assignment. Revise your Change Proposal and Implementation Draft, using feedback from your instructor, and attach it to your paper. Add any necessary transitions to bridge the sections of the paper.
Review your Logic Model, Change Proposal, and Initiation Plan. Describe in detail how the overall Change Plan will be evaluated and the resources needed to evaluate the project. Discuss the evaluation process in relationship to the projected outcomes.
Create a dissemination plan. Explain how the outcome of the project will be disseminated externally (outside the setting to health care community) and internally (unit or hospital where the change process has taken place). A detailed plan answer includes who, what, where, how, and when?
This assignment requires that you support your position by referencing at least three to five scholarly resources. At least two of your supporting references must be from peer-reviewed resources.

Evaluation of the change plan
The use of change theories remains instrumental in altering systems within health care organization systems and the community. In addressing the issue of antimicrobial resistance (AMR) within the hospital setting and in the community, change theories such as innovation diffusion theory and force field and the unfreezing-change-refreezing model will be used. The review of the logic model, change proposal, and the initiation plan will be a necessary step in designing a dissemination plan. For instance, in achieving the project outcomes of reducing disease infections, increasing disease control strategies, and overall prevention of AMR, the project will integrate Roger’s five –steps of innovation diffusion theory and Lewin’s three-steps of change model Wojciechowski, Murphy, Pearsall & French, 2016. As nurses are important to change agents, their skills and behaviors largely influence organizational and community actions throughout the process of change. Therefore, nurses will play a critical role in the change plan both in the evaluation stage and the project dissemination step to ensure the successful implementation of the project.
According to Kurt Lewin’s theory of unfreezing-change-refreezing theory, the behavior is a product of balanced forces that work in opposing directions (Wojciechowski, Murphy, Pearsall & French, 2016). Lewin’s change model depends on both the resistance and driving forces from employees and other nurses oppose or support the proposed change. Roger’s change theory is a modification of the Lewin’s theory to comprise of five steps: knowledge, persuasion, decision, implementation, and confirmation (Mohammadi, Poursaberi & Salahshoor, 2018). The refreezing stage of Lewin’s theory and the confirmation stage of Roger’s model involves finalizing and accepting change as a new habit.
