Part II: Logic Model (1 page)
Include a logic model schematic of the initiative indicating how the program activities and goals are linked. In the diagram, include the purpose, context, inputs, activities, outputs, and outcomes.
Include at least one indicator (specific, observable, measurable) for activities, output, and outcomes that could be used to evaluate your program. Follow instructions on Kellogg Foundation Guide and use the attached slide as a formatting guide.
Part III: Program Narrative (1 page)
Provide a narrative describing the theoretical linkages between the activities and outcomes of the program. Lay out your assumptions around why you believe your program will work, and why your approach will be effective. Address the factors and trends in the larger environment that may influence the program's success or failure.
You will be expected to cite background research supporting your selected activities and how these activities are linked to the intended outcomes within the given environmental context. The narrative should be closely aligned with the logic model schematic.
Part IV: Budget (1 page)
Develop a detailed program budget that clearly outlines the amount of funding needed for each type of expense as well as a description of why each type of expense is needed. State the assumptions made in calculating the budget. In calculating the budget, pay attention to the following: (1) Distinguish between programs in the budget (2) Distinguish between program and non-program expenses The detailed budget for your program should be clear, well organized, easy to understand, and aligned with your program description and narrative. NOTE: Only include an expense budget (not revenue budget), and make sure your total budget does not exceed $500,000.

The organization aims to provide teens and young adults with the paramount, accurate, and unbiased facts needed to make informed decisions, as well as improve upon un-stigmatized and accessible STI/STD testing, treatments, and contraceptives.
What public health needs will this program meet?
This program will help meet the public health need to provide comprehensive sexual and reproductive health education to teenage/young adult populations. By providing teenagers/young adults with accurate and appropriate information for their age, we can prevent unsafe sexual behaviors and provide them with the knowledge to make healthy sexual decisions (Shalihin et al., 2023).
What are the facts and sources that back up the need for your project?
The high prevalence of STD/STI among teenagers and young adults and the increased teenage pregnancy rate in the United States necessitates this project. Sources that back up the need for the project include the Centers for Disease Control (CDC) and Shannon & Klausner’s (2018) journal article titled. According to Shannon & Klausner (2018), approximately 20 million new STIs are recorded annually. Half of these cases occur among adolescents aged 15-24 years. On the other hand, CDC reported the highest number of teen pregnancies in the Southern region of the US. In 2020, Mississippi had the highest birth rate in the US, with 27.9 out of 1,000 births for girls aged 15-19 (CDC, 2020). The other states of concern include Arkansas, Oklahoma, Louisiana, Alabama, and Kentucky, all of which have a birth rate above 23 per 1,000 girls ages 15-19 in 2020 (CDC, 2020). These facts and sources back up the need for this project.
Who will benefit from the project, and how? Define the intended targets of your program.
Teenagers and young adults aged 13-18 will benefit from the project. Through the program, the individuals will learn how to prevent STDs/STIs and early pregnancy by using various protective measures of sex, specifically condoms. They will also learn how to get tested and access treatments and contraceptives without feeling stigmatized.
What activities will be pursued (in the next year) to achieve your objectives?
We plan to cooperate with local hospitals in the communities and hold an annual event that aims to discuss the prevention of STDs and the usage of various protective measures of sex for teens and young adults. The teens and young adults from the local community will be invited as participants. The event will last three days, and we will have a guest speaker each day. We want to invite doctors who are professionals in related topics from local hospitals to deliver the lectures.
