In her report that outlined the readiness of Willow Bend Hospital for accreditation, Diane noted that even though the policy addressing terminology and abbreviations was available, she could not locate the person who was supposed to update the list and ensure that it was disseminated to all the clinical areas, updated in the clinical record system and conducted within definite time frames. This identification contravenes the Joint Commission’s Standard IM.02.02.01, which gives the guidelines that the hospital should use to collect the health information. Diane should consult what the Joint Commission guidelines are and work with the management in order to come up with the acceptable terminologies for use in the hospital. (Bates, 2014).
  2. In her report, Diane also established that there was need to add staff positions to the policy which addressed the auditing process of health records. Even though there had been talk of incorporating the new position of clinical documentation specialist on that committee, this had not yet been formalized. This concern by Diane is contained in the Joint Commissions Standard IM.01.01.01 on information management planning. Section 4 of this guideline points out that the hospital should select staff and licensed independent practitioners who are supposed to take part in the assessment, selection, integration, and utilization of information management systems for the delivery of care, treatment and services. This can be corrected by holding a meeting with the management in order to look for a way of including the newly created position of clinical documentation specialist into the committee to match the guidelines of the Joint Commission Information Standards (Joint Commission Information Standards, 2010).
