Coursework Instructions:

Standardized Procedure for Psychiatric Emergency Worksheet
Assignment
Purpose
The purpose of this assignment is to create a standardized procedure that may be applied to nurse practitioner practice.
This assignment will allow for discovery into the role of the psychiatric mental health nurse practitioner (PMHNP) in providing care for clients experiencing psychiatric emergencies and supports the professional formation of the PMHNP practice role.
Course Outcomes
This assignment enables the student to meet the following course outcomes:
CO 1: Interpret subjective and objective data to develop appropriate diagnoses and evidence-based management plans for patients and families with complex psychiatric mental health diagnoses across the lifespan (PO 5)
CO 2: Formulate appropriate evidence-based management plans of complex psychiatric mental health patients across the lifespan that align with the patient and family goals and perspectives of mental health and wellness (PO 5)
CO 3: Make recommendations based on pharmacologic assessment in the management of complex mental health patients addressing polypharmacy, drug interactions and other adverse events (PO 5)
CO 4: Examine appropriate use of seclusion and restraints in the management of acutely ill psychiatric mental health patients across the lifespan (PO 1)
CO 6: Apply ethical and legal principles to the healthcare needs and management of acutely ill, and complex psychiatric mental health patients across the lifespan (PO 1)
Due Date
The Standardized Procedure Worksheet is due by Sunday 11:59 p.m. MT at the end of Week 2.
The Late Assignment Policy applies to this assignment. Students are expected to submit assignments by the time they are due. Assignments submitted after the due date and time will receive a deduction of 10% of the total points possible for that assignment for each day the assignment is late. Assignments will be accepted, with penalty as described, up to a maximum of three days late, after which point a zero will be recorded for the assignment.
Total Points Possible
This assignment is worth a total of 110 points.
Preparing the Assignment
Nurse practitioners (NPs) provide high-quality, comprehensive care to clients in a variety of health care settings. Standardized procedures may be used to help guide care. Boards of Nursing, such as CaliforniaLinks to an external site., provide resources to ensure that NPs provide quality care while functioning within their scope of practice.
Select one of the conditions or disorders below and create a standardized procedure for treatment of a psychiatric emergency.
serotonin syndrome
Follow the guidelines below to complete the standardized procedure templateLinks to an external site..
Definition
Identify the selected disorder or condition.
Provide a basic description of the pathophysiology of the selected disorder or condition.
Provide a summary of the incidence and prevalence of the selected disorder or condition in the United States.
Assessment
Identify common subjective findings (symptoms) consistent with the selected diagnosis.
Identify pertinent physical examination components and the expected exam findings consistent with the selected disorder or condition.
Identify your intended state of practice and whether physician collaboration is required, as it is in California. If collaboration is required, describe the circumstances that would require physician consultation.
Diagnostic tests
List diagnostic tests appropriate for the selected disorder or condition.
List expected abnormal results consistent with the selected disorder or condition.
Management: Present an evidence-based management plan for the selected disorder or condition.
Describe in detail recommended treatments for the selected disorder or condition, including medical and psychiatric management strategies.
Identify first-line prescribed medications recommended for the selected disorder, if any.
Discuss expected changes to treatment after client is stabilized.
Describe expected client follow-up. If the client called in with persistent or recurrent symptoms after initial treatment, describe the appropriate next action.
Describe indications for referral.
Use current clinical practice guidelines or U.S.-based scholarly resources appropriate for advanced practice* that has been published within the past 5 years to support management plan
Important Note: Textbooks, journals published outside the United States, and journals not intended for advanced practice nurse or physician providers are NOT considered scholarly references for treatment decisions.

Name:
Carefully read the assignment guidelines and rubric and complete each section of the worksheet below.
1. Definition
a. Disease or condition:
Serotonin syndrome
b. Pathophysiology
The disorder called "serotonin syndrome" is brought on by the body having too much serotonin. This can occur when two or more medications that affect serotonin levels are taken together, when certain medications are taken with dietary supplements, or when certain drugs are taken in large doses (Francescangeli et al., 2019). Serotonin is a neurotransmitter that helps to regulate feelings of wellbeing, mood, appetite, and sleep. There are a number of symptoms that can develop when the body produces too much serotonin. These are: agitation, increased blood pressure, increased heart rate confusion, muscle twitching, diarrhea and sweating. In severe cases, serotonin syndrome can lead to seizures, coma, or even death (Scotton et al., 2019). Serotonin syndrome's precise pathophysiology is not yet fully understood. It is thought that the excess of serotonin causes changes in the regulation of neurotransmitter activity in the brain and other body systems. This leads to a disruption in the normal balance of nerve signals, leading to the symptoms of serotonin syndrome.
c. Incidence and prevalence
The incidence and prevalence of serotonin syndrome is not well known. It is estimated that between 0.1% and 0.2% of people taking medications that can cause serotonin syndrome develop the condition, although the actual number may be higher (Ott et al., 2019). The condition is believed to be more common in people taking multiple medications that affect serotonin levels, such as antidepressants, and in those taking higher doses of these medications. It is also thought to be more common in people who are also taking other drugs that can interact with serotonin-affecting medications.
2. Assessment
a. Symptoms
When certain drugs that alter the body's serotonin levels are used simultaneously, Serotonin Syndrome, a potentially fatal illness, may develop (Talton, 2020). Serotonin syndrome symptoms can be moderate to severe and can include: Agitation, restlessness and confusion, sweating, high blood pressure and rapid heart rate, dilated pupils, headache, tremors or twitching muscles, diarrhea, loss of coordination, heavy feeling in the arms and legs, muscle rigidity, shivering, fever and seizures.
b. Physical exam findings
Physical exam findings for serotonin syndrome can include: Mental status changes (such as agitation, confusion, hypomania, and coma), tachycardia (rapid heart rate), hypertension (high blood pressure), hyperthermia (elevated body temperature), myoclonus (involuntary muscle contractions), tremor, diaphoresis (excessive sweating), hyperreflexia (exaggerated reflexes), clonus (a series of repetitive and rhythmic muscular contractions), ocular clonus (rapid, involuntary eye movements), dilated pupils, flushing, hypertonicity (increase in muscle tone), gastrointestinal symptoms (such as nausea, vomiting, and diarrhea) (Francescangeli et al., 2019)
c. Intended state of practice and circumstances that require physician consultation, if indicated
In the state of Georgia, physicians s...
