Insufficient funding for hospitals, specifically university hospitals directly affect the efficiency of their operations. With the increasing number of patients who are in dire need of special attention, the availability of funding for such programs has been on the decline. University Healthcare handles variety of applications that ranges from inpatient and outpatient services, emergency healthcare and also research. To avoid the domino chain of reaction that is associated with diminished funding for such programs, university hospitals need to come up with innovative ways to raise funds to cover the deficit and ensure operations are going on smoothly.
Various solutions are available to solve the problem. One is to maximize on Medicaid waivers granted by the state. Medicaid waivers are granted by the Center for Medicaid services to states to help for piloting innovations and children health insurance program. Waivers are not only used by these hospitals to expand coverage and services but also to demonstrate innovative service delivery systems, which improve care, increase efficiency and reduce cost. Also known as the Triple Aim (Berwick, Nolan, & Whittington, 2008). With the growth in cost and complexity of healthcare a wider scope for Medicaid waiver is seen as a solution to the little funding program.
One of the implemented programs is the Texas 1115 waiver also known as transformation and quality improvement program. The program uses uncompensated care pool to reimburse hospitals for uncompensated care cost. Moreover, the delivery system reform incentive payment (DSRIP) gives health care providers incentives for successfully implementing Center for Medicaid and Medicare approved projects. Such projects must achieve milestone related to improving the structure and redesigning the delivery of attention.
Texas program is anchored on university healthcare system. The fund ensures the university health care system can offer quality care within Texas, manage and prevent chronic conditions, provide access to medical and dental care. Additionally, the funds must enable the hospitals to integrate behavioral health services, provide prenatal and preventive pediatric care and device mechanism of lowering the high rates of communicable disease such as tuberculosis. Pooling of funds by the state ensures that primary care expansion is met, chronic care management and patient navigation. More so most of the projects integrate behavioral health into the mainstream care system.
