Patient:
Estela, an active, socially inclined 52-year-old Hispanic woman, was brought to the emergency department (ED) for severe abdominal pain with nausea and vomiting. During history taking, Estela also mentioned changes in her bowel movement. She recently stopped going out with her friends due to frequent diarrhea. She was known diabetes with uncontrolled blood glucose level and on hemodialysis every M-W-F schedule. She was admitted in the hospital and for treatment of her blood sugar and further work up of her abdominal pain.
Few days following admission, a comprehensive medical history, complete physical exam and tests were taken. Estela was diagnosed with irritable bowel syndrome (IBS). Since there is no cure for IBS, multifaceted therapeutic approach was initiated for Estela. Aside from medications for IBS that are based on her most troublesome symptoms, she was also started with serotonin-norepinephrine reuptake inhibitor (SNRI).

Different factors can influence a patient's pharmacokinetics and pharmacodynamics and control their psychiatric medication. These factors include age, ethnicity, gender, and behaviors (Sáez‐Belló et al., 2021). Age affects this process in that it is associated with a reduction of the first-pass metabolism. People characterize older adults by reduced kidney function, reduction in liver mass, and blood flow, which affects the movement and interaction in the body. Gender is another factor in the pharmacokinetics process in women is affected by gastrointestinal motility and lower body weight than men. Also, women are more likely to experience an unfavorable drug reaction than men. They are also prone to diseases like depression and cataracts. This shows that women in psychiatric medication will be treated differently to men because sex differences have crucial implications for medication activity. Ethnicity affects this process in that the genetic differences between ethnic groups are responsible for drug metabolism. Behaviors like depression and anxiety problems also affect the movement and interaction of drugs in the body.
