Pathological process in the development and presentation of eczema versus psoriasis
Keratinocytes of eczema (atopic dermatitis) patients exhibit a proneness to the overproduction of chemokines and cytokines. This condition can probably lead to triggering and maintenance of the inflammation process. The body's immune response against the environmental allergens makes the dendritic cells in the affected skin play its role in amplifying allergen-specific T cell responses. Then, there is cross-linkage of IgE on the mast cells of the dermis, which initiates the release and synthesis of mediators. Mediators activate eosinophils, leading to tissue damage, hence permanent inflammation of atopic dermatitis skin, presented by red and itchy skin (Hinks et al., 1987).
In contrast, psoriasis occurs due to cellular changes in the skin. There is hyperplasia of the epidermal keratinocytes cells in the skin, hyperplasia of the blood vessels, and infiltration of the leucocyte (T lymphocytes and neutrophils) in the affected skin.
