Muscarinic antagonists and agonists are regularly used to regulate immoderate sweating and cure hyperhidrosis (excessive sweating). In addition, muscarinic agonists such as atropine are used to treat diseases caused by extreme activities of muscarinic receptors. In contrast, muscarinic antagonists are used to cure disorders caused by a lack of sufficient muscarinic receptors or none (Pleuvry, 2004). Muscarinic agonists are divided into five classified as; M1, M2, M3, M4, and M5. M1, M3, and M5 are transmembrane receptors that join together to form the G9 protein, which upregulates phospholipase C (PLC) (Toai, 2021). In addition, the PLC sticks to phosphatidylinositol 4, 5-biphosphate (PIP2) into 1,2diacylglycerol (DAG) and Inositol 1, 4, 5, triphosphate (IP3). DAG activates protein kinase C, which activates the downstream protein and causes calcium influx. Additionally, IP3 causes the sarcoplasmic reticulum to discharge stored calcium. Thus the increased intracellular calcium enhances the contraction of muscles and exocrine glandular secretion.
On the other hand, M2 and M4 receptors are Gi receptors that obstruct adenylyl cyclase. The inhibition reduces adenosine 3'5', monophosphate (cAMP) production from ATP. Therefore, a decrease in cAMP concentration consequently decreases the activation of Kinase A protein. (Toai, 2021)
