Although families often present with one person identified as the “problem,” the assessment process will help you better understand family roles and determine whether the identified problem client is in fact the root of the family’s issues.

CC (chief complaint): Patti presented her case because her household was experiencing chaos. She had a problem with depending on her children and expecting them to take care of her in her health condition. She seeks to lead her own life but expects her children to understand that they should be there for her as per her customs. She feels her children are avoiding her and does not want to spend quality time with her since all four are concentrating on their own lives (Bacigalupe, 2006, 09:45).
HPI: P. is a 40-year-old Iranian female who presents herself for psychotherapeutic evaluation for hopelessness and helplessness. She is recuperating from surgery on her legs, rendering her paralyzed and needing support for her daily activities. However, she is not on psychotropic medication at the moment. After her surgery, she was referred to a psychiatrist for medical attention and evaluation, considering that she displayed helplessness and hopelessness and felt no longer in charge of her children as they were out of control (Bacigalupe, 2006, 07:50). The symptoms above depicted started when her daughter emigrated from Iran to the United States to join the rest of the family. However, chaos erupted in her household owing to the physical and sexual the girl, Sheela, underwent at the hands of her father. P. now feels like her children are avoiding her, and she cannot control them as it should be, thus feeling hopeless and helpless. The symptoms persisted for a long time until she saw the psychiatrist, Sandi. The frequency of the symptoms is very high, as they arise daily. Moreover, they have affected her mental health greatly as she feels she receives less and wants more done (Bacigalupe, 2006, 09:35).
Past Psychiatric History:
General Statement: The patient entered the treatment two years before meeting with Gonzalo after chaos erupted in her household with counseling for hopelessness and helplessness (Bacigalupe, 2006, 04:45).
Caregivers (if applicable): The patient has no history of hospitalization or medication trials for the psychotropic problem.
Hospitalizations: There are no records of the patient's hospitalization apart from the two surgeries conducted on her legs. Also, neither has the patient ever received any detox or residential treatment before the counseling nor has any history of suicidal or self-harm behaviors.
Medication trials: the patient presents no history of psychotropic medications.
Psychotherapy or Previous Psychiatric Diagnosis: the patient's psychiatric diagnosis before the evaluation with Sandi, another psychiatrist. The patient gained an understanding of the distinction between individualist and collective living, although it was hard to understand the growth and detachment of the children (Bacigalupe, 2006, 08:35).
Substance Current Use and History: The patient has no history of substance use.
Family Psychiatric/Substance Use History: Her family does not have any individual with recorded evidence or instance of substance use.
