This paper is to reference my concept analysis paper on Evidence-based practice.
This is a reflective and creative piece where you will be developing your own middle range theory based on your specific area of practice and/or interest.
In this final paper, you will need to apply what you have learned through your course readings, concept analysis paper, and discussions.
How I suggest you proceed:

Open your Concept Analysis paper from week six and recall your selected concept.
Reflect on what you learned about your concept and how it relates to your area of practice. Ask yourself, "what needs further explanation related to the concept and my practice setting? Is there something amiss about the way the concept is being used, applied, and understood in my practice setting? Why is this happening?"
Using your concept, create a clinical question based on your specific area of practice. For example, Mishel’s Uncertainty Theory was developed to explain how patients cognitively process illness-related stimuli and construct meaning in these events.
Next, decide on a second concept for your theory. This concept could also come from the research done on your concept analysis paper. It may be extrapolated from your discussion on the concept’s uses, attributes, antecedents, or consequences. Just ensure that the two concepts (that will make up your theory) are related and inform your clinical question. Mishel (see attached article) used several concepts and components in her theory development. You are only required to use two concepts.
Next, define and describe each of your concepts. Each definition and description should come from what you have learned and experienced at your practice setting. You may write in first person. Each concept needs one clear definition and description.
Next, describe the relationships between the two concepts. Relationships are the links among and between concepts. You should have some ideas about the relationship between the two concepts based on what you have learned through your Concept Analysis research and what you have noticed in your practice setting. The way that relationships emerge provides theoretic purposes and assumptions on which your theory is based. Reflect on the nature of the relationships between and among your concepts. Are the relationships descriptive or do they explain something about the phenomenon of interest (clinical question)? No need to create figures. Just be descriptive in your discussion about the relationships.
This is not a rewrite of your Concept Analysis. This paper is about theory construction/development based on what you learned about your concept as it relates to your current practice setting and your clinical question.
Next, describe what role research played in the development and use of your theory in practice. In other words, was research vital in the development of your theory? Explain how. Also, how do you see research supporting the application of your theory to practice? Provide an example.
Lastly, describe barriers to the use/application of your theory. Was your theory developed for a specific area of practice or specialty, or is it generalizable? Provide support for your answer.
Write 5- 7 pages (not including title and reference pages) using APA 7th edition. No limit on references (you should have plenty from previous coursework) and some new ones. The title page must include the name of your theory.
Your level one headings should be: Concepts & Clinical Question, Definition and Description, Relationships, Research & Application of Theory, Barriers to Application of Theory, and Conclusion.

The development of theories plays a significant role in nursing. According to Im (2018), nursing theories guide nursing practice and research. They provide a hypothesis that professionals can test through research before it can be applied in practice. This paper's aim is to develop a middle-range theory that can be used in home care/nursing. Home nursing services have become prominent in recent times due to changes in demographics as well as the nature of diseases. Yet, home nursing services are provided in settings not initially designed for patient care, making it challenging to apply evidence-based interventions that would typically be used in a healthcare setting (Melby et al., 2018). Given the importance of EBP in improving the quality of care and patient health outcomes, it is crucial to explore the concept of EBP in the home nursing services setting.
Clinical Question and the Second Concept
Specifically, the following clinical question will be explored: How can home care professionals adapt evidence-based practices to settings that initially were not designed for patient care? This clinical question will also be informed by a second concept, namely knowledge transfer. This concept is closely related to the concept of EBP, as will be explained further in the concept definition section.
Concept Definitions
The theory will be based on assumptions from two key concepts: EBP and knowledge transfer.
EBP
EBP refers to a structure used by healthcare professionals to guide their clinical judgment/decisions when assessing and treating patients (Fulcher-Rood et al., 2020). Healthcare professionals use EBP to incorporate the best evidence from research, clinical experience and expertise, and patient preferences into patient care. The EBP process comprises six steps: assess, ask, acquire, appraise, applicability, and act (Dilard, 2017). These steps ensure that healthcare professionals stay current on new evidence for improved patient care.
The critical components of EBP include the clinical question being identified, obtaining the best research related to the clinical question, clinical experience, considering the patient as a whole, and implementing the new practice (Dillard, 2017; Hardy, 2018; Walker & Avant, 2019; Hsieh & Chen, 2020). Healthcare professionals must be knowledgeable about EBPs. This knowledge is necessary for implementing EBP, especially in non-traditional healthcare settings such as home settings where some adaptations are required. When the critical components of EBP are considered in any healthcare setting, and healthcare professionals are knowledgeable about EBP, it becomes easier for them to provide quality and safe care that improves patient outcomes.
