Mrs. J. is a 63-year-old married woman who has a history of hypertension, chronic heart failure, and chronic obstructive pulmonary disease (COPD). Despite requiring 2L of oxygen/nasal cannula at home during activity, she continues to smoke two packs of cigarettes a day and has done so for 40 years. Three days ago, she had sudden onset of flu-like symptoms including fever, productive cough, nausea, and malaise. Over the past 3 days, she has been unable to perform ADLs and has required assistance in walking short distances. She has not taken her antihypertensive medications or medications to control her heart failure for 3 days. Today, she has been admitted to the hospital ICU with acute decompensated heart failure and acute exacerbation of COPD.
Subjective Data
Is very anxious and asks whether she is going to die.
Denies pain but says she feels like she cannot get enough air.
Says her heart feels like it is "running away."
Reports that she is exhausted and cannot eat or drink by herself.
Objective Data
Height 175 cm; Weight 95.5kg.
Vital signs: T 37.6C, HR 118 and irregular, RR 34, BP 90/58.
Cardiovascular: Distant S1, S2, S3 present; PMI at sixth ICS and faint: all peripheral pulses are 1+; bilateral jugular vein distention; initial cardiac monitoring indicates a ventricular rate of 132 and atrial fibrillation.
Respiratory: Pulmonary crackles; decreased breath sounds right lower lobe; coughing frothy blood-tinged sputum; SpO2 82%.
Gastrointestinal: BS present: hepatomegaly 4cm below costal margin.
Intervention

This is a case of Mrs. J., a 63-year-old female patient, who has a chief complaint of difficulty breathing. The condition started three days ago when the patient suddenly experienced flu-like symptoms, including fever, productive cough, malaise, and nausea, with associated difficulty in performing the activities of daily living, mainly when walking for short distances. The patient complains of difficulty breathing, palpitations, anxiety, and difficulty performing tasks.

The recommended therapy for patients with heart failure and reduced ejection fraction (HFrEF) includes the combination of an angiotensin-converting enzyme inhibitor (ACEI) or angiotensin-receptor neprilysin inhibitor (ARNI), beta-blockers, and mineralocorticoid receptor antagonist (MRA). The patient’s symptoms should be observed after the administration of these drugs. ARNI is preferable over ACEI in cases where the first three mentioned drugs did not alleviate the patient’s symptoms. Additionally, sodium-glucose co-transporter 2 (SGLT-2) inhibitors, namely, dapagliflozin or empagliflozin, are recommended, except where the said drugs are contraindicated (McDonagh et al., 2021).
The nursing interventions for the cardiovascular problems at the time of admission included an ACEI (Enalapril), beta-blocker (metoprolol), and intravenous loop diuretic (furosemide). Based on the clinical guidelines, the patient should also be given spironolactone or eplerenone, MRAs, and SGLT-2 inhibitors. Additionally, since the patient shows signs of peripheral hypoperfusion as manifested by low blood pressure, the nurse should administer an inotropic. If there is relief of congestion and hypoperfusion signs and symptoms, then the medical therapy should be continued. If there is no relief of congestion, the clinician may increase the loop diuretic dose or combine it with another diuretic. Subsequently, if there is no relief from congestion and hypoperfusion, the clinician should prescribe vasopressors (McDonagh et al., 2021; Yoshioka et al., 2022).
ACEIs are used to reduce patients’ overall mortality and morbidity with HFrEF. Beta-blockers can be used in combination with ACEIs to reduce mortality and morbidity and control atrial fibrillation (Sreenivas, 2021; McDonagh et al., 2021). MRAs prevent aldosterone from binding to its receptors, effectively maintaining blood pressure and preventing congestion (McDonagh et al., 2021). Lastly, SGLT-2 inhibitors also reduce mortality in patients with HFrEF (Honigberg et al., 2020).
The patient is also presenting with signs and symptoms of acute pulmonary edema. The management for this case is oxygen support in cases where the peripheral oxygen saturation is less than 90%, provided by the nursing intervention and loop diuretics (McDonagh et al., 2021).
