BACKGROUND
This week, we examine a 31-year-old male who presents to the office with a chief complaint of insomnia.
SUBJECTIVE
Patient is a 31-year-old male. He states that his insomnia has gotten progressively worse over the past 6 months. Per the patient, he has never been a “great sleeper” but is now having difficulty both falling asleep and staying asleep at night. The problem began approximately 6 months ago after the sudden loss of his fiancé. The patient states this is affecting his ability to perform his job, which is a forklift operator at a local chemical company. The patient states he has used diphenhydramine in the past to sleep but does not like the way it makes him feel the morning after. He states he has fallen asleep on the job due to lack of sleep from the night before. The patient's medical record from his previous physician states that he has a history of opiate abuse, which began after he broke his ankle in a skiing accident and was prescribed hydrocodone/apap (acetaminophen) for acute pain management. The patient has not received a prescription for an opiate analgesic in 4 years. The patient states recently he has been using alcohol to help him fall asleep, approximately four beers prior to bed.
MENTAL STATUS EXAM
The patient is alert and oriented to person, place, time, event. He makes good eye contact and is dressed appropriately for time of year. He denies auditory/visual hallucinations. Judgement, insight, and reality contact are all intact. Patient denies suicidal/homicidal ideation, and is future oriented.

Introduction 
The case study focuses on a 31-year-old Male with insomnia, one of the most common sleep or wake disorders where there are sleeping difficulties at night or waking up several times at night. Inadequate sleep causes a person to feel fatigued, and stressful situations and altered emotional states worsen insomnia, but chronic insomnia can result from other underlying health issues. There are also difficulties concentrating and undertaking usual daily activities, which can affect the quality of life. Chronic (long-term insomnia) represents insomnia for more than three weeks, and the client’s insomnia symptoms have worsened over the past six months. 
There is a need to monitor the client closely as he reported he uses alcohol to fall asleep, and alcohol intake may interfere with drug absorption and the medications’ effectiveness. He is also taking hydrocodone/APAP (acetaminophen) to manage acute pain management and had previously used diphenhydramine to sleep better but was uncomfortable with how he felt in the morning. Diphenhydramine is an antihistamine that is potentially harmful in large amounts.
Sleep hygiene, which represents healthy sleep habits, improves the quality of life, and sleep hygiene is combined with medication where necessary to treat insomnia. Sleep hygiene improves sleep quality and changes lifestyle such as restricting screen time drinking caffeinated beverages before bed. Other activities, such as relaxation, light exercises, and having a regular daily hygiene routine, promote better sleep.  Regular bedtime and time to wake up help establish regular patterns and rhythms, but the environment where an individual sleeps ought to be quiet and with adequate lighting temperature. The three decisions are to start Trazodone 50 mg PO at bedtime, decrease Trazodone to 25 mg daily at bedtime, and then continue dosage while encouraging sleep hygiene and follow up in four weeks.
Decision #1 
Trazodone 50 mg PO at bedtime is the preferred choice in the first decision point. Trazodone is a selective serotonin reuptake inhibitor with low side effects. The medication is FDA-approved for depression and treats primary and secondary insomnia (Stal, 2014). There are no concerns about dependence or withdrawal when using Trazodone, but the side effects are dose-dependent (Jaffer et al., 2017). In a meta-analysis from Yi et al. (2018), the researchers highlighted that Trazodone had been found to reduce the number of early awakenings. The medication is efficacious and well-tolerated. Trazodone is also an antidepressant with sedating properties that improve sleep, but Trazodone should not be dosed fully as an antidepressant when treating and managing insomnia. 
The other choices, “Zolpidem 10 mg daily at bedtime” and “Hydroxyzine 50 mg daily at bedtime”, were not chosen.
