Statement of Problems

 Opioid addiction is affecting daily functioning and worsening the client’s health.
Opioid addiction continues to be a problem, but decrease followed by recovery from drug addiction.
The substance abuse problem is also linked with a risk of the co-occurring disorders worsening and the client has previously been diagnosed with
anxiety
When the client has no access to the opioids there are withdrawal symptoms
The client is concerned that opioid has affected his relations with his close family members who are also concerned with his drug use
It is challenging to carry out daily functioning for the client addicted to opioid

Statement of Objectives

Integrate the individual therapy plan with the substance abuse treatment program to improve outcomes in a client using opioids.
Improve treatment engagement after evaluation
 Evaluate the psychological and emotional wellbeing
Client will be able to identify his symptoms and understand the effects of opioids addiction
