  • Review the patient’s past and current medications. Refer to Stahl’s Prescriber’s Guide and consider medications you might select for this patient.

  • Review the posttest for the case study.

  • Explain what physical exams and diagnostic tests would be appropriate for the patient and how the results would be used.

  • List three differential diagnoses for the patient. Identify the one that you think is most likely and explain why.

  • List two pharmacologic agents and their dosing that would be appropriate for the patient’s sleep/wake therapy based on pharmacokinetics and pharmacodynamics. From a mechanism of action perspective, provide a rationale for why you might choose one agent over the other.

  • If your assigned case includes “check points” (i.e., follow-up data at week 4, 8, 12, etc.), indicate any therapeutic changes that you might make based on the data provided.

Introduction
A case of a woman who is 42 with an intricate psychiatric history that involved insomnia, visual hallucination, posttraumatic stress disorder, impaired aggressive behavior, and depression. In the duration of 36 months, the patient was treated with Tiagabine, Paroxetine, Remelton, and Bupropion. A combination of Bupropion, Quetiapine, and Lexapro was also given to the client. The medical history of the patient consists of GERD, overweight, CAD, diabetes, Glaucoma, OSA, SOPD, and drug use.
Questions Relevant to Patient’s Condition with Rationale
There are chances of having signs of the mental problem because of certain medicines. Another possibility of mental illness is broad medical history and consumption of several medications. Few relevant questions are as follows:
1 Have you ever take herbal treatment and I would check for connections with present medication procedure?
2 The albuterol inhaler has few after-effects such as insomnia, anxiety, and nervousness (Birur et al., 2017). For that reason, I need to confirm how many times do you use albuterol inhaler?
3 What about your sleep schedule, is it regular sleep or not? as the patient is suffering from insomnia so I need to know about sleeping schedule.
People in the Patient’s Life
In the present case study, it seems that the patient may not be a trustworthy source to provide accurate information regarding health, medical history, and present condition. The main reason is her mental illness that affects her efficacy. For that reason, the following questions from her family members would be asked.
The question one would be raised from the patient’s family members along with her psychotherapist to get precise information about actual reason for mental illness. Moreover, another question from the patient’s family members/psychotherapist would be the previous medical meetings or psychiatric records to identify the cause of mental issues.
Physical Exams and Diagnostic Tests
1 Physical exam of the thyroid is essential because many patients with mental illness are suffering from thyroid disease. It is recommended to get a diagnostic test report of Thyroid Stimulating Hormone (TSH). As per current research, it partially confirms that a person who is suffering from hypothyroidism shows depressive signs (Jucevičiūtė et al., 2019). The diagnostic report will be helpful to cure thyroid disease for a better mental condition.
