Subjective:
CC (chief complaint): The patient has drawn concern to his supervisor, who sets him up for the interview due to poor concentration at work. He is concerned that he is not able to deliver work with the same energy he had initially, and he feels he is the only one not able to meet deadlines at this time. The things he initially knew how to do are becoming more challenging by the day. At home, he becomes messy, forgetting important items such as shoes and his phone. He even needs to pay his bills on time.
HPI: The patient's name is Harold Brown. A sixty-year-old male who is Caucasian. He presents to the clinic to evaluate his current change in attention and input at work. The supervisor has referred him for further review and assessment out of concern for how he has had trouble concentrating at work. He has had difficulty with attention and delays in remembering important events. He cannot perform simple tasks the way his job requires him to, yet he was able to perform these tasks initially. He also finds concentrating difficult and often misses out on valuable information. Additionally, he is messy and has become quite forgetful. His symptoms started not long ago and have significantly affected his life at work and home. Therefore, the symptoms indicate a severe problem that must be addressed as soon as possible before it escalates to a complete lack of cognitive function.
Past Psychiatric History:
General Statement: The patient had never had any mental health treatment, even when the symptoms were clearly present when he was a child. Further, he has never had any prior evaluation for mental illness.
Caregivers (if applicable): Not applicable
Hospitalizations: The patient has not mentioned any hospitalization incident related to his psychiatric health.
Medication trials: The patient has not mentioned an account for any psychotropic medication he has had previously.
Psychotherapy or Previous Psychiatric Diagnosis: The patient might be aware that he suffers from a mental health problem due to recent events and occurrences at his workplace. He is very responsive, which shows that he is willing to get any necessary remedy for his situation. As mentioned earlier, there is no previous diagnosis of his psychiatric health, nor are there treatments.
Substance Current Use and History: The patient denies any history of drug use. However, he admits that he takes a scotch drink with a cigar every weekend. Additionally, he takes caffeine which helps him to stay calm throughout his day.
Family Psychiatric/Substance Use History: The patient has not mentioned any family history of psychiatric disorders.
Psychosocial History: There is no mention of where the patient was born or who raised him. However, it is mentioned that he has a little brother though nothing else has been said concerning the brother. He is not married and does not have children. He, however, relates closely with other people in casual dating. He has a bachelor’s degree in engineering and has no listed hobbies.
