The scenario selected involves a 7-year-old boy who has had asthma all his life. Several treatments have been tried, but none work completely. An ethical dilemma exists on whether to place him on a new medication that has been developed and yet to be approved for people under 12. Ethical and legal guidelines should always be applied before prescribing any drug. In this scenario, the practitioner should consult JJ's parents and family, give them all details about the new medication and seek their consent on whether JJ should proceed with the said medication.

Various stakeholders are involved in making a judgment for the case scenario. The prescriber, the nurse practitioner, is the first person to commence the treatment plan. Ethical and legal standards dictate that the prescriber should be fully aware of prescription guidelines for children of all ages and adults. Ignoring prescription guidelines for the new medication will place the prescriber in an ethical and legal battle. The pharmacist follows the prescriber in that they are responsible for dispensing the medication. In this scenario, the pharmacist should be aware of all the ethical and legal implications of starting JJ on the new medication, considering that he is a child below 12 years.
