Part A: Executive Summary
The executive summary condenses your entire grant proposal into one page. It introduces a problem, describes a program that will successfully address that problem, and briefly highlights the financial resources the program will require. Some funders only skim the executive summary, so write it first and last (edit heavily when the full proposal is completed).
Write a 1-2 page executive summary of your program proposal. It should include:
• a sentence or two that outlines needs or a problem
• an overview of the program you are proposing, and how that program will satisfy the need (i.e. why the program is imperative)
• your program objectives and evaluation benchmarks
• a quick overview of about how much the program will cost.
• a conclusion that thanks the funder for their consideration of your request.
Part B: Logic Model
Logic Models provide a visual representation of how the program you propose will create change in the world. Logic Models help grant writers consider the resources that will go into a program and the short and long term outputs they are hoping for.
Create a Logic Model for your Program Proposal. Use a template that provides a good form for what you want to communicate. Make sure it includes the following components:
• Program Title
• Inputs- Describes resources invested in the program
o Examples: Staff, volunteers, time, money, materials, equipment.
• Activities -Describes what the program does. Be sure to use the present tense: Train, convene, inspect, test, etc.
o Examples: Provides counseling sessions, conducts workshops on heart disease, inspects housing units, distribute smoking cessation materials.
• Outputs- Measures what the program does and who the program reaches.
o Examples: # of workshops held, # of brochures distributed, # of schools visited, # of parents served, # of counseling sessions conducted, # of days shelter is provided to individuals without a home.
• 1 Short Term Outcome-Describes individual, family, or community change that can be measured (like changes in learning such as attitude, knowledge, or skills).
o Examples: Adults living in a fraternity house in Athens, Ohio increased their knowledge by 30% on how to correctly identify that a smoke detector is functioning by June 30, 2017.
• 1 Benchmark to measure Short Term Outcomes (i.e. a Measure or Indicator). Outcome measures should be specific, and require data collection.
o Example: % of fraternity members improving their knowledge of how to correctly check that a smoke detector is functioning on a pre- and post-test evaluation.
• 1 Long-Term Outcome. Describes outcomes that affect a larger community and that take a longer time to happen.
o Example: Reduced unintentional injuries and deaths from house fires in Athens County.
• 1 Benchmark to measure Long Term Outcome. Larger scale change that can be measured or that will be indicated on data that is already being collected.
• Example: Rate of hospitalization and mortality from house fires in Athens County
Part C: Program Goals & Objectives
Interpret your Logic Model from a visual form into a verbal one. How would you summarize your program’s key goals (long term) and objectives (short term). What are indicators that will let you know you are being successful?
Write a 1-2 page narrative that answers the following questions:
• What are the program’s goals and objectives?
• How will the outputs and outcomes that will result from the intervention or proposed program be measured?
• Make sure your objectives are:
o Specific
o Measurable
o Achievable
o Relevant
o Time-bound
