The cognitive disorders distort the thought process of the client; Delirium is a temporary condition that alters the level of consciousness; hence, it can affect the focus thought, memory, understanding and the use of language (Eby & Brown, 2008). The condition can be caused by the metabolic disorders, nutritional disorders, metabolic states, psychosocial condition or from substance-related conditions. The condition can be reversed if the underlying cause is treated. The response to delirium requires the collaboration of the nurses, their assistants, and the physicians. The screening procedure should involve thorough physical assessment, the mental status and the complete history of the client. The physical restraint of the client should be used under special considerations. Nurses are in the best position to identify the changes that might be happening in the cognition and the behavior of the client.
