As frontline healthcare caregivers, nurses must obtain the patient’s health history as a prerequisite for delivering quality and safe patient care. The obtained health history often features subjective and objective data from the patient, creating an appropriate basis for the patient care team to create an equally effective patient care plan for achieving the desired patient outcomes. Provided herein is a patient’s health history highlighting the diverse information and data obtained from patients and its importance in delivering safe and quality care.
Health History: Subjective Data
The patient or participant is a 55-year-old African American man living with his wife and two children in a middle-income neighborhood where he runs a medium-sized mowing company. The patient presented an increased rate of or frequent urination, fatigue, blurred vision, numb feet, and increased hunger and thirst over the past week, prompting him to seek medical attention CITATION Doy15 \l 1033 (Doyle, 2015). The outlined signs and symptoms are indicative of type 2 diabetes. Due to the outlined symptoms, he finds it difficult to discharge his normal daily operations, such as conducting meetings with clients and employees for his mowing company. The participant was concerned about his deteriorating health and would like to receive quality and safe patient care to continue managing his business and taking care of his family. Further inquiry into the patient’s medical history revealed that he had no past medical conditions, including recent hospitalizations or surgical procedures, only that he was trying to quit the unhealthy habit of cigarette smoking. The patient’s family has a history of type 2 diabetes suffered by his late grandmother and an uncle from his father’s side.
The general review of systems confirmed the participant’s presentation of fatigue and troubled sleeping, with the endocrine study highlighting increased thirst and frequent urination. There were no signs of any cardiac complications, but the patient pointed out occasional cramping of legs and calf pain while walking during the vascular review. He further confirmed experiencing vision changes, including blurred and double vision. The neurological assessment showcased numbness on his feet and tingling in his hands, with the respiratory review indicating shortness of breath. The participant belongs to the old age stage of psychosocial development as he exudes knowledge and wisdom in his concern for his health and the overall wellbeing of his family. He is also married to an African American woman, with the entire family being staunch Christians who believe in taking care of themselves and seeking medical attention whenever it is needed. The middle-income society they live in further promotes individual and collective awareness of personal and public health concerns. The community boasts of an active public health organization that sensitizes society members on diverse health issues, such as dealing with chronic health problems and the importance of embracing healthy lifestyle habits.
