Accident Human Factors Analysis

Each student is to research and submit a written assignment report of the following accident involving aircraft VH-ZCR and the circumstances associated with the loss of control and collision with terrain reported by the Australian Transport Safety Bureau (ATSB) in its Aviation Occurrence Investigation, AO-2017-024 Final dated 24 Sept 2018.

Following the format in Table 1, the report is to provide your analysis of what occurred and discuss salient gaps and deficiencies from a human factors perspective using Reason’s Swiss Cheese Model and any other 1 of the other following prescribed human factor models (SHELL model, SCHELL model, PEAR model, Dirty Dozen model, HFACS model). If appropriate, the paper should also indicate any resultant changes to aviation regulations and legislation.
The paper should be in your own words as much as possible; where you need to quote from other sources you must properly acknowledge and reference those sources.
The first document is the analysis report by Australian Transport Safety Bureau.
The second document is the Harvard references template.
